阻塞性睡眠呼吸暂停(OSAS)是认知功能障碍的独立危险因素,circRNA通过竞争性结合miRNA参与认知功能障碍的发生。前期研究发现,miR-138上调和组蛋白去乙酰化酶(sirtuin 1)的表达,诱导海马神经元细胞衰老,促进阿尔茨海默病的发生。生物信息学预测circHDAC9与miR-138-5p互成碱基配对,而circRNA在外泌体中含量丰富,预实验结果提示,OSAS合并认知功能障碍患者血浆外泌体circHDAC9表达减少。因此我们猜想,OSAS下血浆外泌体传递的circHDAC9表达减少,导致miR-138-5p升高,衰老因子表达增加,引起海马神经元细胞衰老,导致认知功能障碍。本研究从临床标本、体外细胞及动物模型探讨外泌体circHDAC9通过miR-138-5p调控认知功能障碍的分子机制,为OSAS相关认知功能障碍的防治、诊断分子标志物的筛选提供新思路。

Obstructive sleep apnea (OSAS) is an independent risk factor for cognitive dysfunction, and circRNAs are involved in the occurrence of cognitive dysfunction through competitive binding of miRNAs. Previous studies have found that miR-138 up-regulates the expression of sirtuin 1, inducing senescence of hippocampal neurons and promotes the occurrence of Alzheimer's disease. Bioinformatics predicted that circHDAC9 was paired with miR-138-5p by mutual nucleotide, and circRNA is abundant in exosomes. The results of pre-experiment suggest that plasm exosomal circHDAC9 expression is decreased in patients of OSAS combined with cognitive dysfunction. Therefore, we hypothesized that the decreased expression of circHDAC9 transmitted by plasma exosomes under OSAS led to the increase of miR-138-5p which could lead to the senescence of hippocampal neurons and cognitive dysfunction by increasing the expression of senescence factors. In this study, the molecular mechanism of exosomal circHDAC9 in cognitive dysfunction by regulating miR-138-5p from clinical specimens, in vitro cell and animal models, providing a new idea for the prevention and treatment of OSAS-related cognitive dysfunction and the screening of molecular markers for diagnosis.
阻塞性睡眠呼吸暂停(obstructive sleep apnea ,OSA)是目前最常见的睡眠疾病之一。OSA的特征是反复出现完全和/或不完全上呼吸道阻塞,表现为睡眠期间反复气流中断(呼吸暂停)或减少,临床症状表现为白天嗜睡、头痛头晕、疲倦乏力、夜间打鼾、多动不安、夜尿增多甚至憋醒等。间歇缺氧(intermittent hypoxia ,IH)是睡眠呼吸暂停的标志,它与肿瘤的发生发展密切相关。我们探索了OSA促进非小细胞肺癌(NSCLC)发展的可能机制。在研究中,我们将非小细胞肺癌(non-small cell lung cancer ,NSCLC)细胞分别暴露于IH或常氧(room air, RA)中,随后提取和鉴定外泌体,发现IH条件下外泌体分泌增多。将外泌体与巨噬细胞一起孵育,检测到IH下 M2型巨噬细胞标志物的表达升高,从而促进NSCLC细胞的增殖、侵袭和转移。构建裸鼠肿瘤发生模型,检测外泌体miR-106a-5p对M2巨噬细胞极化和NSCLC细胞恶性的影响。我们的研究结果表明,IH外泌体促进M2巨噬细胞的极化,从而促进NSCLC细胞的增殖、侵袭和转移。通过对信号通路的探索,我们发现miR-106a-5p通过NSCLC分泌的外泌体转移到巨噬细胞内,通过下调PTEN和激活STAT3信号通路促进M2巨噬细胞极化,从而促进肿瘤的发生发展。对于NSCLC合并OSA患者,血清外泌体miR-106a-5p水平与AHI呈正相关。.同时我们还发现IH条件下,骨髓来源的骨间充质干细胞 (BMSC) 分泌的外泌体与促进肿瘤发展有关。相比于RA,IH促进BMSCs分泌更多的外泌体,进一步促进肺腺癌细胞转移和侵袭。通过对信号通路的检测,我们发现IH促进BMSCs外泌体内miR-31-5p表达增多,进一步促进WDR5的表达,促进上皮细胞间质化改变,从而促进肺腺癌进展。外泌体miR-106a-5p,MiR-31-5p代表了伴随癌症和NSCLC患者潜在治疗靶点。
