膜迷路积水是梅尼埃病(MD)基本病理特征。研究表明MD患者球囊功能受到影响,导致颈性前庭诱发肌源性电位(cVEMP)振幅耳间不对称性改变,但与积水量不呈线性相关,机制不明,研究其机制有利于明确膜迷路积水对球囊功能的影响,但因涉及学科交叉,积水机制研究一直是耳科的一大难题。根据高清晰度微CT图像,申请人已建立正常人全耳力学模型,通过前期研究发现,球囊积水量和球囊斑振幅比非线性相关,这可能是膜迷路积水时cVEMP异常的原因,而球囊与镫骨足板的空间非对称性可能是其生理基础。因此,本项目拟在完善前期建立的全耳力学模型基础上,结合颞骨和动物试验,研究球囊和镫骨足板空间非对称性对膜迷路积水下球囊功能的影响机制。再根据MD患者内耳钆造影磁共振结果,建立个性化模型,并将仿真结果和患者cVEMP结果进行关联性分析,明确膜迷路积水对球囊功能影响的机制,以及膜迷路积水和cVEMP的关联,为MD诊治提供理论依据。
英文摘要
Endolympatic hydrops was the basic pathological feature of Ménière’s disease (MD). Studies showed that the saccular function of MD patient was impaired. Thus the cervical vestibular evoked muscle potential (cVEMP) was affected, that the interaural amplitude difference ratio was changed, however it was non linear to the volume of endolymph. The mechanism was unknown; however, the revealing of the mechanism would clarify how endolymphatic hydrops affect the saccular function, which was difficult to study due to its interdisciplinary characteristic. According to high resolution micro CT, a whole human ear mechanical model was constructed, and we found that, the volume of the endolymph and the displacement of the macula sacculi was nonlinear, which may be the reason of the abnormal cVEMP, and the spatial asymmetry between the saccule and the stapes footplate may be the physical basis. As a result, current study would base on the refinement of our previous whole ear finite element model, along with temporal bone and animal model experiments to study the spatial asymmetry between the saccule and the stapes footplate on the saccular function under endolymphatic hydrops. Then individual model would be established based on individual patient’s MRI, and the results would be compared with the cVEMP, aimed to clarify the mechanism of the affection of endolymphatic hydrops to the saccular function, and the link between the endolymphatic hydrops and cVEMP, provide theoretical reference to clinical interventions of MD.
