基于“补肾健脾方”增强IFN-γ介导的非溶细胞抗病毒效应探讨"扶正"治则在慢乙肝治疗中的作用机制

治则是指导治疗方法的总则，“扶正”是我们广泛应用的治则之一，那么现代医学背景下如何阐明其科学内涵，成为了推动中医理论不断前进的必然。治则治法的科学阐释，离不开以具体疾病为靶点的验证和适宜病种的选择。慢性乙肝是危害人类健康的重大传染病，目前尚无治愈手段，免疫耐受和缺陷是HBV不能彻底被清除的原因所在，而“扶正”的中医内涵就是扶助正气、提高机体抗邪能力，恰恰针对乙肝发病机理。HBV清除方式主要有：CTL介导的溶细胞机制和IFN-γ等细胞因子介导的非溶细胞性机制，后者不损伤正常肝细胞，无疑具有更大的临床价值，也是目前研究的热点。“扶正”与“祛邪”不同，作用温和不伤人体正气，与非溶细胞免疫病毒清除方式又有切合之处。因此，本项目以“扶正”治则指导下的“补肾健脾方”为干预手段，探讨其是否可调节IFN-γ介导的抗病毒通路，增强IFN-γ对HBV的清除，从而阐明“扶正”治则的分子机制和作用靶点。

Treatment principle is the general rule to guiding treatment method. “Fu Zheng” is one of the rules that we apply widely. How to clarify its scientific connotation of it, has become a necessity to promote the theory of Chinese medicine. The scientific interpretation of therapeutic principle and method is inseparable from the verification of specific diseases and the selection of suitable diseases. Chronic hepatitis B is a major infectious disease that endangers human health，but there is no cured method at present. Immune tolerance and defects are the reasons why HBV cannot be completely eliminated. The connotation of Chinese medicine of “Fu Zheng” is to help the righteousness and improve the body's ability to resist evil just for the pathogenesis of hepatitis B. CTL cytolytic mechanism and IFN-γ-mediated non-cytolytic mechanisms are the main methods of eliminating the HBV. There was no damage on normal liver cells of non-cytolytic mechanisms, which undoubtedly has greater clinical value and research value. the effect of “Fu Zheng” is gentle and does not hurt the human body is different from “Qiu Xie”, but consistent with non-cytolytic immune virus clearance. Therefore, this project uses the “Bushen Jianpi Recipe” under the guidance of “Fu Zheng” rule to explore whether it can regulate the IFN-γ-mediated antiviral pathway and enhance the clearance of HBV by IFN-γ, thus clarifying the molecular mechanism and target of the “Fu Zheng” principle.
