血管性认知障碍出现的学习、记忆、理解、执行能力下降,严重干扰患者日常生活。2型糖尿病是其重要的危险因素,近年来伴2型糖尿病的血管性认知障碍发病机制及治疗方法研究日益成为研究热点。针康法在脑血管病各功能障碍的治疗中发挥重要作用,课题组前期研究中已从影像学角度证实了针康法可改善缺血性卒中患者的认知功能。基于前期研究结果,我们推测针康法在改善与认知相关脑区功能的同时,对其结构亦会产生影响,同样也会对与2型糖尿病和认知功能高度相关的血清生物标志物产生影响。本研究拟以VBM为主要分析方法,以SOD、MDA、BDNF为主要观察指标,通过动物实验与人体试验,从细胞形态学及影像学角度探讨针康法对合并2型糖尿病的血管性认知障碍大鼠及患者海马体积的影响,及其对海马组织及外周血清中氧化应激损伤的干预机制,为针康法治疗合并2型糖尿病的血管性认知障碍提供可靠的实验基础,并阐明其神经保护机制的科学内涵。

The ability of learning, memory, understanding and execution of vascular cognitive impairment is decreased, which seriously interferes with the daily life of patients. Type 2 diabetes mellitus is an important risk factor of vascular cognitive impairment. In recent years, the pathogenesis and treatment of vascular cognitive impairment with type 2 diabetes mellitus have become a research hotspot. Acupuncture-rehabilitation therapy plays an important role in the treatment of various dysfunction caused by cerebrovascular diseases. In the previous study, the research group has confirmed that Acupuncture-rehabilitation therapy can improve the cognitive function of patients with ischemic stroke from the perspective of imaging. Based on the results of previous studies, we speculated that Acupuncture-rehabilitation therapy could not only improve the brain function related to cognition, but also affect its structure, as well as the serum biomarkers highly related to type 2 diabetes mellitus and cognitive function. In this study, voxel-based morphometry was used as the main analysis method, SOD, MDA and BDNF were used as the main observation indexes. Through animal experiments and human experiments, the effect of Acupuncture-rehabilitation therapy on the volume of hippocampus in rats and patients with vascular cognitive impairment combined with type 2 diabetes mellitus and its intervention mechanism on oxidative stress injury in hippocampus and peripheral serum were discussed from the perspective of cytomorphology and imaging. The treatment of vascular cognitive impairment with type 2 diabetes mellitus provides a reliable experimental basis and elucidates the scientific connotation of its neuroprotective mechanism.
