慢性难愈合性创面因其治疗周期长、用药选择少,至今仍是临床医学的巨大挑战。藤苦参为傣族传统用药,外用治疗烂疮,促进创面愈合功效确切,但其物质基础和作用机制尚未见报道。申请人前期对藤苦参的化学成分进行了系统的研究,发现强心甾类化合物是其主要成分;对其中一个主要的强心甾成分洋地黄毒苷元进行了初步研究,发现洋地黄毒苷元可促进创伤大鼠的创面愈合,且该功效可能与其激活Na/K-ATPase/Src信号通路有关。基于此,申请人提出科学假说:“藤苦参强心甾成分可激活Na/K-ATPase/Src信号通路,调节成纤维母细胞的增殖、迁移和胶原合成过程,是藤苦参促进创面愈合的药效物质基础”。本项目拟在系统分析、制备藤苦参强心甾成分的基础上,阐明其促进创面愈合的作用机制,并对外用藤苦参促进创面愈合的临床安全性进行评价,指导其合理应用,为民族药物藤苦参用于治疗慢性难愈合性创面提供科学依据。

Chronic and unhealed wounds is still one of the major clinical challenges in the world due to its long treatment cycle and insufficient clinical treatment options. Streptocaulon griffithii Hook. f. (S. griffithii), a folk medicine plant of the Dai minority, has long been used as a topical treatment of chronic wounds due to its wound healing activity. However, the material basis and action mechanism of its wound healing activity has not been reported yet. Our previous study showed that cardiotonic steroids (CTSs) were the main components in S. griffithii. Further study of digitoxigenin, one of the main CTSs derived from S.griffithii showed that digitoxigenin could promote the wound healing of rats and the action mechanism may be the activation of the Na/K-ATPase/Src signaling pathway. Based on this, a hypothesis was raised up that “The material basis of the wound healing activity of S. griffithii was CTSs and the action mechanism of the wound healing was the regulation of the proliferation, migration and the collagen synthesis process in fibroblast via the activation of the Na/K-ATPase/Src signaling pathway”. In this study, the CTSs in S. griffithii will be analyzed and the typical ones will be isolated. Then, the action mechanism of the wound healing activity of S. griffithii will be elucidated and the safety of topical administration of S. griffithii for the treatment of chronic wounds will also be evaluated. Results of this study will provide scientific basis for the clinical rational use of S. griffithii for the treatment of chronic and unhealed wounds.
