Rosacea is well established as a chronic inflammatory skin disease, whose pathophysiology remains largely unclear. Abnormalities in the innate immune system and neurovascular dysregulation have been considered to be primarily implicated in the development of rosacea, and over-production of cathelicidin LL37 plays a key role in these processes. Our previous data showed that mTORC1 signaling is hyperactivated in skin lesions (especially in epidermis) of both rosacea patients and mouse model. By employing LL37-induced rosacea mouse model, we showed that mTORC1 deletion in epithelium or pharmacological inhibition significantly alleviated the rosacea-like phenotypes. Conversely, hyperactivation of mTORC1 signaling exacerbated the development of rosacea. Moreover, we demonstrated that by binding to TLR2, cathelicidin LL37 regulates mTORC1 signaling through a positive feedback circuit in keratinocytes. So, we anticipated that mTORC1 signaling is pivotal in the pathophysiology of rosacea. Here, by using an inducible conditional gene targeting strategy to ablate mTORC1 in adult skin epithelium, we will further figure out the role of mTORC1 signaling in the development of rosacea. We will clarify the positive feedback circuit between mTORC1 and cathelicidin LL37 through both gain- and loss-of-function studies and protein mass spectrometry and co-immunoprecipitation. Then, using RNA-sequencing analysis, we will unearth differentially expressed genes and signaling pathways in skin lesions of rosacea patients and mouse models. The function and regulatory mechanisms of those genes or pathways will be further identified in vivo and in vitro. In summary, our findings will support mTORC1 signaling as a novel determinant in rosacea and hence a potential new therapeutic target.
背景:玫瑰痤疮是一种好发于中青年女性的慢性炎症性皮肤病。目前认为遗传易感性、皮肤屏障损伤、免疫紊乱以及神经血管功能失调是参与该病发生发展的重要环节。在这些环节中表皮抗菌肽LL37的过度表达发挥关键作用,然而其具体的生成调控机制及后续作用机制仍不清楚。另外由于迄今为止尚没有发现与玫瑰痤疮发病密切相关的明确靶点,目前的治疗方法大多是基于临床症状的对症治疗,只能暂时缓解症状,无法治愈,这严重影响患者的身心健康。.主要研究内容:通过转录组学分析发现,多条免疫相关的信号通路在玫瑰痤疮患者中高度富集,其中包含mTORC1信号通路。通过免疫组化、免疫荧光等手段确定mTORC1信号通路在玫瑰痤疮患者和小鼠模型皮损中的表达情况。通过观察皮损表现、检测组织病理学情况以及相关基因表达来评估mTORC1信号的缺失和过表达对小鼠玫瑰痤疮样皮炎发生的影响。体外研究主要是在人角质形成细胞中进行,通过免疫印迹分析,免疫荧光染色、RT-qPCR和免疫共沉淀等手段对mTORC1信号通路活化的具体机制进行了深入探究。.重要结果及关键数据:(1)mTORC1信号通路在玫瑰痤疮中过度活化;(2)mTORC1信号的失活可减轻LL37诱导的小鼠玫瑰痤疮样皮炎表型,另外, mTORC1的过度激活可促进且加重小鼠玫瑰痤疮样皮炎表型的发生发展;(3)在人角质形成细胞中,mTORC1可与抗菌肽LL37形成正反馈调节环路,在该环路中抗菌肽LL37通过与受体TLR2结合激活mTORC1信号,而mTORC1的活化又可进一步促进抗菌肽LL37的生成,从而产生过量的LL37;(4)上述环路中产生的过量的LL37通过mTORC1信号进一步诱导NF-κB信号通路的活化以及玫瑰痤疮相关的细胞因子和趋化因子的产生,从而放大炎症反应,即级联瀑布效应,最终导致玫瑰痤疮的发生发展;(5)初步临床试验证实mTORC1特异抑制剂雷帕霉素外用可改善玫瑰痤疮患者临床症状。.科学意义:揭示了mTORC1信号通路与LL37之间形成的正反馈环路在玫瑰痤疮的发病中发挥着重要作用,是一个潜在的新的治疗靶点。此外,mTORC1-LL37形成的增强放大调节环路也可能是玫瑰痤疮难以治愈的原因。
