“上病下治”与现代医学“脑-肠”轴学说不谋而合,以此为基础的“肠源性代谢物-中枢小胶质细胞”互动反应,有望成为揭示中医通腑法治疗中风科学内涵的关键突破口。中风发生后,肠源性色氨酸代谢紊乱,其代谢/合成关键酶IDO1/WARS1异常表达,通过TREM-1/TREM-2/SYK放大炎症信号,加重中枢炎性损伤。大黄作为中医通腑法治疗中风的要药,临床上广泛用于中风急性期患者的治疗。课题前期研究发现大黄可显著改善肠道菌群结构及肠道通透性,下调中风后IDO1、TREM-1及SYK的表达。结合以上工作基础,本项目提出以“肠源性色氨酸代谢-中枢小胶质细胞极化”为桥梁,大黄可通过调节IDO1/WARS1等色氨酸代谢关键酶,进而影响肠源性色氨酸代谢及其下游TREM-1/SYK信号传递,最终改善神经炎症,发挥抗脑缺血作用的科学假说。有望完善大黄治疗急性期中风的作用机理,进一步揭示中医通腑法治疗中风的科学内涵。
英文摘要
"Upper disease, lower treatment" coincides with the theory of "brain-gut" axis in modern medicine. The interactive reaction of "gut derived metabolites-central microglia" based on this theory is expected to become a key breakthrough to reveal the scientific connotation of traditional Chinese medicine Tongfu method in the treatment of stroke. After stroke, the metabolism of intestinal tryptophan is disturbed, and IDO1/WARS1, the key enzyme of metabolism/synthesis, are abnormally expressed. The inflammatory signal is amplified by TREM-1/TREM-2/SYK, which aggravates the central inflammatory injury. Rhubarb is widely used in the treatment of patients with acute stroke as an important medicine in the treatment of stroke. Previous studies found that rhubarb can significantly improve the compositions of intestinal flora and intestinal permeability, and down-regulate the expression of IDO1, TREM-1 and SYK after stroke. Based on the above results, this project proposes a scientific hypothesis that rhubarb can regulate the expressions of IDO1/WARS1, and then affect intestinal tryptophan metabolism and its downstream TREM-1/SYK signal transmission, ultimately improve neuroinflammation and play an anti-cerebral ischemia role by taking "intestinal tryptophan metabolism central microglia polarization" as a bridge. It is expected to enrich the mechanism of Rhubarb in the treatment of acute stroke, and further reveal the scientific connotation of “Tongfu” traditional Chinese medicine in the treatment of stroke.
党的十八大以来,习近平总书记多次就中医药工作作出重要指示,强调要注重用现代科学解读中医药学原理,要加强研究论证,说明白,讲清楚中医药的疗效。践行总书记重要指示精神,采用现代科学方法阐明中医药的药效物质基础及作用机制,是推进中医药现代化进程的重要手段。中医药治疗脑卒中的优势显著。大黄具有泻下攻积,清热泻火,凉血解毒,逐瘀通经等功效,是基于中医通腑理论治疗IS的常用中药,临床疗效确切,但其治疗IS的药效物质基础和作用机制尚未完全阐明。选择中医药优势病种缺血性脑卒中及大宗中药大黄为研究范例,评价大黄治疗缺血性脑卒中的药效,并采用微生物组学、色氨酸靶向代谢组学和分子生物学等方法探索大黄的作用机制。结果证实大黄能够显著降低MCAO模型大鼠的神经功能缺陷评分、脑组织水肿率、脑梗死体积和局部脑血流下降率,改善脑组织病理变化,治疗IS的药效显著。 大黄能够通过改善MCAO大鼠肠道微生物组成和肠黏膜屏障功能,发挥治疗IS的作用。 大黄能够通过调节色氨酸代谢途径,降低细胞IDO-1酶活力,大鼠血清sTREM-1含量及大鼠结肠色氨酸代谢关键酶IDO-1、WARS1及下游免疫信号调控关键因子TREM-1的表达,发挥治疗IS的作用。 大黄能够通过抑制TREM-1/p-SYK、CARD9/NF-κB和NLRP3/ASC/Caspase-1信号通路,调节小胶质细胞极化,以及降低促炎因子的表达,升高抗炎因子的表达,抑制神经炎症反应,发挥治疗IS的作用。 大黄酚-8-0-β-D-葡萄糖苷和儿茶素没食子酸酯、莲花掌苷和芦丁能够与IDO-1结合,抑制TREM-1表达,可能是大黄调节肠源性色氨酸代谢,改善肠粘膜屏障的潜在活性成分;没食子酸甲酯、大黄酚-8-0-β-D-葡萄糖苷和大黄酸能够与TREM-1结合,抑制小胶质细胞NLRP3表达,从而抑制神经炎症,可能是大黄抑制神经炎症的潜在活性成分。
