慢性鼻窦炎(CRS)是最常见的呼吸道疾病之一,造成巨大社会经济负担。最新研究发现中性粒细胞胞外诱捕网(NETs)在CRS中扮演重要角色,但机制不明。申请人前期研究发现:1)CRS患者鼻黏膜同时存在NETs和上皮-间质转化(EMT),且标志物水平呈正相关;2)测序提示NETs调控TGF-β通路;3)EMT相关通路中仅TGF-β1/Smad通路上调。据此,提出科学假设:NETs调控TGF-β1/Smad通路诱导鼻上皮发生EMT。本项目拟扩大样本量检测鼻黏膜NETs、EMT和TGF-β1/Smad通路水平,确认NETs与EMT及CRS疾病严重程度的关系;通过细胞分子实验明确CRS组织微环境诱导NETs形成,以及NETs诱导EMT发生的作用;通过调控TGF-β1/Smad通路活性,阐明NETs诱导鼻上皮发生EMT的分子机制。本课题有望为CRS的发病机制提供新的理论依据,以期发现新的生物学治疗靶点。

Chronic rhinosinusitis (CRS) is one of the most prevalent chronic respiratory diseases, which imposes immense socio-economic burden. Recent observations strongly suggest that neutrophilic extracellular traps (NETs) may play an important role in the pathogenesis of CRS. However, the pathogenic mechanisms remain largely unclear. In the preliminary experiments, we observed both NETs and epithelial-mesenchymal transition (EMT) in the nasal mucosa of CRS patients, and found a positive correlation between NETs and EMT markers. RNA-seq analysis of the clinical samples identified a potential relationship between NETs and TGF-β pathway in CRS patients. Moreover, the up-regulation of transforming growth factor-β1 (TGF-β1)-Smad signaling pathway was observed in the nasal mucosa of CRS patients, while no significant alteration was detected in other EMT-related pathways. Therefore, we postulate that NETs may induce EMT in CRS by targeting TGF-β1/Smad signaling pathway. In this study, the relationship between NETs and EMT levels, as well as that between NETs and the severity of CRS will be determined by detecting NETs, EMT markers, and TGF-β1/Smad pathway molecules in the nasal mucosa in a larger CRS cohort. In addition, in vitro functional experiments will be carried out to confirm the stimulation of NETs formation by the microenvironment of CRS tissues, and the induction of EMT by NETs. Finally, the mechanisms underlying the induction of EMT by NETs in nasal epithelium will be explored through modulation of the activity of TGF-β1/Smad pathway. This project may provide new insights into the pathogenesis of CRS and lead to novel therapeutic targets to ameliorate the disease.
本研究旨在探讨慢性鼻窦炎(CRS)患者鼻窦黏膜中中性粒细胞胞外诱捕网(NETs)与上皮间质转化(EMT)的相互作用及其机制。通过免疫荧光染色和多抗体共标记技术,发现CRS患者鼻窦黏膜中同时存在NETs和EMT标记物阳性的细胞,且其表达强度呈正相关。定量检测显示,CRS患者中NETs和EMT相关标志物的表达水平显著升高,且二者正相关。为了验证鼻黏膜上皮免疫微环境对NETs形成的影响,我们建立了外周血中性粒细胞提取与培养体系,并通过鼻黏膜上皮组织与中性粒细胞共培养,观察到CRS患者免疫微环境显著促进了NETs的形成。尽管TGF-β1表达显著升高,但Smad2、Smad3和Smad7表达无明显差异,提示TGF-β/Smad通路之外的机制可能参与了NETs对EMT的诱导作用。为进一步探讨NETs对EMT的调控作用,我们采用单细胞RNA测序技术分析了NETs阳性和阴性各5例CRS患者的鼻窦黏膜样本。结果发现,NETs阳性组中Cluster 2亚群的上皮细胞数量显著增加,且这些细胞具有独特的生物学特性:DLK2、FGFR3和S100A2基因上调,提示其具备较强的分化与增殖能力;MMP10、BCAM和S100A2的表达增强,表明细胞通过降解细胞外基质增强迁移能力,促进鼻黏膜重塑;IL33和ISLR显著上调,增强上皮细胞的炎症反应和免疫应答;ALDH3A1和FMO2的高表达有助于增强细胞清除有害物质及抗氧化应激能力。体外实验验证显示,NETs通过DLK2-Notch通路激活EMT,抑制该通路后,EMT表型显著逆转,表明DLK2-Notch信号通路在NETs诱导的EMT过程中发挥关键作用,为深入探讨NETs在CRS中诱导EMT过程的机制提供了新的理论依据。
