咽鼓管的主动开放受腭帆张肌、腭帆提肌甚至咽鼓管咽肌等咽鼓管旁肌的控制,但各肌的开放效能及协调尚存争议。声导抗等技术仅能检测鼓室静止状态以间接反映咽鼓管功能,而咽鼓管声测法以声波记录咽鼓管主被动开放过程,是实时动态反映咽鼓管开闭的可靠手段。我们前期已研发了数字化咽鼓管声测仪,并在内镜下实现了上述各肌肌电图同步采集,同步咽鼓管旁肌肌电图-声测法技术已获得正常人数据,初步提示了各咽鼓管旁肌的作用。另一方面,鼻咽癌放疗常引起咽鼓管功能障碍和各种中耳炎,研究拟采用该同步技术追踪放疗前、中、后的咽鼓管功能、咽鼓管旁肌电活动及咽鼓管周粘膜病变,以期阐明肿瘤及放疗所致咽鼓管相关神经源性、肌源性及粘膜性损伤的时机、程度和机制,指导防治。放疗对各咽鼓管旁肌造成的随机单一损伤还将形成功能阻断模型,适于探讨各肌效能及协调机制。研究还拟对非放疗的各种中耳炎进行对照实验,阐明放疗损伤和炎症因素对咽鼓管功能的影响。

The active opening of the eustachian tube (ET) is controlled by paratubal muscles, the extensor palatini muscle, the levator palatini muscle and even the salpingopharyngeus muscle. However, the opening efficiency and coordination of each muscle are still controversial. Sonotubometry records the active and passive opening process of the eustachian tube by sound wave. Unlike acoustic impedance technique, detecting only the static condition of the tympanum, indirectly reflects the ET function, sonotubometry is a more reliable technique, which dynamically reflects the opening and closing of the ET in real time. We have developed a digital sonotubometry device in previous studies, and achieved simultaneous electromyography (EMG) collection of multiple paratubal muscles under endoscope. The cross-platform data of synchronous paratubal EMG and sonotubogram was integrated using MATLAB. A Study of normal volunteers suggested the role of each paratubal muscle. On the other hand, nasopharyngeal carcinoma (NPC) usually causes post-radiotherapeutic dysfunction of ET and various kinds of otitis media. In order to clarify the timing, extent and mechanism of neurogenic, myogenic and mucosal injuries of ET induced by radiotherapy, our synchronous cross-platform technique is to be used to follow NPC patients, before, during and after radiotherapy, for the function of ET, the activity of paratubal muscles and the mucosal lesions surrounding the ET orifice. The potential random irradiation injury to the paratubal muscle might provide a muscle dysfunction model, which is helpful in exploring the efficacy and coordination mechanism of each muscle. To differentiate the effects of radiation injury and inflammatory factor on the function of ET, non-radiotherapy-induced otitis media cases are also involved in this study as a control.
