肌腱移植重建前交叉韧带(ACL)术后,关节腔内移植物塑形不良(松散、血管化差、甚至吸收)是影响手术疗效的关键因素,其机制不明。我们前期研究发现:塑形不良的移植物胶原合成代谢及MKX基因表达明显下调,而尿源性干细胞外泌体可以减轻缺氧微环境介导的ACL成纤维细胞(ACLFs)线粒体氧化应激损伤,促进ACLFs胶原代谢稳态的维持。因此,我们提出“MKX基因修饰及ACLFs特异性适配体功能化的尿源性干细胞外泌体局部靶向ACLFs调控胶原代谢稳态,促进移植物塑形改建”的科学假说。本研究拟: 1)构建MKX基因修饰及ACLFs特异性适配体功能化的尿源性干细胞外泌体;2)体内外观察新型外泌体在缺氧微环境对ACLFs胶原稳态的调控效应;3)预测并验证新型外泌体调控ACLFs胶原稳态的关键分子及下游靶基因,构建移植物胶原代谢稳态调控的通路网络。本课题的顺利开展将为临床ACL移植物塑形提供新的思路。

The poor remodeling of intra-articular tendon graft (loosening, poor vascularization,resorption, ) after anterior cruciate ligament (ACL) reconstruction is the key factor that affects the outcome of the operation,but the underlying mechanism is not clear. Our previous study found that the collagen anabolism and MKX expression of poorly remodeled graft were significantly down-regulated.The urine-derived stem cell exosomes can diminish the oxidative stress damage to ACL fibroblasts (ACLFs) mitochondria mediated by internal hypoxic microenvironment, which is beneficial to the collagen metabolism homeostasis of ACLFs. Therefore, we hypothesized that MKX gene-modified and ACLFs-specific aptamer-functionalized urine-derived stem cell exosomes would locally take ACLFs as target and regulate the collagen metabolism homeostasis of ACLFs, thus to improve the remodeling outcomes of the tendon graft. In current study,we intend to : 1) construct the MKX gene-modified and ACLFs-specific aptamer-functionalized urine-derived stem cells exosomes; 2)observe the modulation effect of the novel exosomes regulating the collagen metabolism homeostasis of ACLFs under hypoxic microenvironment in vivo and vitro; 3)predict and verify the key biomolecule and downstream target genes and construct the modulation pathway of collagen metabolism homeostasis of graft. The result of the study will provide a new treatment strategy for the poor ACL graft remodeling.
本项目通过构建MKX 基因修饰及ACLFs 特异性适配体功能化的尿源性干细胞外泌体(Apt-USCs-MKX-Exos),探索新型外泌体对缺血缺氧状态的ACL肌腱移植物塑形的影响及对体外氧糖剥夺的ACLFs细胞线粒体功能及胶原代谢的作用及机制。本项目研究发现,Apt-USCs-MKX-Exos促进体外氧糖剥夺的ACLFs线粒体及线粒体内嵴形态和数量的稳定、提高细胞缺氧耐受能力、增强胶原合成代谢、抑制胶原分解代谢;同时,Apt-USCs-MKX-Exos可改善关节内腔内缺血缺氧状态的移植物ACLFs的线粒体功能,保护线粒体结构完整,提高移植物缺血坏死期线粒体及线粒体内嵴数量,防止线粒体“空泡“样变性,促进ACL移植术后胶原稳态的维持,提高ACL移植物胶原纤维平均直径及大直径胶原纤维的比例,改善胶原纤维排列、血管浸润及胶原纤维生物力学强度。其作用机制可能为:Apt-USCs-MKX-Exos激活线粒体铁离子运输和呼吸相关共济蛋白(FXN),促进线粒体ATP合酶(ATP5F1A)、及活线粒体抗氧化酶(SOD-2)的表达,改善ACLFs中线粒体能量供应,进而上调ACLFs中肌腱调节蛋白(TNMD)的表达,激活TGF-β/Smad2通路,促进胶原合成代谢相关基因(Col1a1、COl3al、Fibronectin、Tenascin-C、TNMD、DCN)表达,抑制胶原分解代谢相关基因(MMP-2、MMP-9、MMP-13、p38、Erk1/2)的表达,有助于实现移植物缺血坏死期胶原代谢稳态,从而促进ACL移植物塑形。本研究阐明了Apt-USCs-MKX-Exos促进ACL肌腱移植物塑形的效应和机制,并发掘了新的干预治疗靶点,为ACL塑形组织工程化外泌体的进一步转化应用及新型分子靶点治疗思路的开发奠定了依据。
