α-突触核蛋白病(α-Syn病,帕金森病等)难以逆转,明确其保护因素及机制对防治意义重大。我们前期研究发现低运动量的特发性快眼动睡眠期行为障碍患者(iRBD,α-Syn病前驱期)更快进展为α-Syn病。预实验发现:运动是α-Syn病的保护因素;运动提高纹状体(α-Syn病核心部位)PI3K/Akt通路相关基因表达;而脂联素(运动可诱发其高表达)基因与α-Syn病显著关联。既往研究提示脂联素受体1(AdipoR1)/PI3K/Akt通路激活有神经保护作用。但运动延缓α-Syn病的机制不明。据此,我们提出“AdipoR1/PI3K/Akt通路介导运动延缓α-Syn病进展”的假说。本项目将通过iRBD队列研究、动物及细胞模型实验,阐明运动提高脂联素水平进而激活AdipoR1/PI3K/Akt通路延缓α-Syn病进展的作用机制。预期结果为延缓/阻断α-Syn病进展的干预措施开发提供理论和实验依据。

α-synucleinopathies (e.g., Parkinson's disease) are difficult to be reversed, and it is of great significance to clarify their protective factors and underlying mechanisms for their prevention and treatment. Our previous study found that patients with idiopathic REM sleep behavior disorder (iRBD, the prodromal stage of α-synucleinopathies prodromal phase) with lower exercise level progress to α-synucleinopathies faster. Our preliminary experiments suggested that 1) high exercise level is a protective factor of α-synucleinopathies; 2) exercise can increase the expression level of some proteins of the PI3K/Akt pathway; and 3) adiponectin gene variation is significantly associated with α-synucleinopathies. In addition, previous studies have found that exercise can significantly increase the level of adiponectin, and that the activation of the adiponectin receptor 1 (AdipoR1)/PI3K/Akt pathway has a protective effect on α-synucleinopathies. However, the underlying mechanisms of the protective effects of exercise on α-synucleinopathies progression remain unclear. Therefore, we hypothesize that "exercise increases adiponectin and subsequently activates the AdipoR1/PI3K/Akt signaling pathway to delay the progression of α-synucleinopathies". This project will employ iRBD cohort, animal and cellular model to clarify that the alleviation effect of exercise on α-synucleinopathies progression is mediated by the increased adiponectin level and its subsequent activation of the AdipoR1/PI3K/Akt signaling pathway. This project will clarify the pathogenesis of α-synucleinopathies and pave a way for the development of preventive and intervention strategies for α-synucleinopathies.
快速眼动睡眠行为障碍(RBD)被认为是帕金森病、路易体痴呆、多系统萎缩等α-突触核蛋白病的特异性前驱期信号。本项目围绕RBD及其相关的神经变性疾病,聚焦于运动与疾病风险预测的关系,从临床和动物实验两个维度展开深入研究。本项目结合横断面和队列研究设计,首次将RBD前驱期人群纳入研究范围。研究发现,运动减少这一表征在前驱期RBD的超早期阶段即已显现,与后期转化为α-突触核蛋白病的风险密切相关。横断面研究揭示了前驱期RBD个体较对照组表现出更长的白天小睡时间、更高的KAR以及显著降低的平均每周活动量等特征。前瞻性队列研究进一步表明,较高的运动水平和活动振幅显著降低了帕金森综合征首次转化的风险(HR范围:0.44-0.49)。这些结果为疾病的早期干预提供了关键线索,并提示运动可能作为延缓α-突触核蛋白病进展的保护性因素。在动物实验方面,本项目深入探讨了运动对脂联素分泌及其作用机制的影响。研究发现,运动显著提升了脂联素这一抗神经变性和抗衰老因子的水平,且该效应存在时点依赖性:在小鼠活跃期(关灯后3小时,ZT15)运动的效果显著优于非活跃期(开灯后3小时,ZT3)。此外,研究团队成功构建了脂联素Adipoq基因敲除小鼠模型,并开展了脂联素受体激动剂AdipoRon的干预实验,证实脂联素及其下游通路在运动延缓神经变性及器官衰老中的核心作用。基于上述研究发现,已获批软件著作权1项,撰写2篇论文,其中1篇获得《Nature Aging》审稿意见(Major revision),并在进一步完善中,另一篇在投稿中。本研究的科学意义在于:(1) 首次明确运动减少是RBD前驱期的早期表征,扩展了运动与神经变性疾病关系的认识;(2) 提出了运动干预的时间效应,为优化运动疗法提供了新视角;(3) 提供了脂联素作为潜在治疗靶点的实验依据,推动了针对神经变性疾病的精准干预策略开发。
