瘙痒是特应性皮炎(AD)最突出的临床症状,具有温热刺激及夜间加重的特征,控制瘙痒是治疗AD的重要基础。热敏瞬时受体电位(TRPV3)通道异常是介导角质形成细胞释放瘙痒介质导致热觉痒化的重要环节。中医理论认为“热微则疮痒”,清热药为止痒常用药,以清热药物为主的清热祛湿方可明显改善急性期AD皮疹及夜间瘙痒。基于前期研究,提出以下科学假说:清热祛湿方可通过TRPV3通道蛋白调控角质形成细胞释放瘙痒介质,改善温热介导的瘙痒。本项目构建TRPV3高表达的细胞和动物模型。采用免疫印迹、流式细胞、RT-qPCR,搔抓实时定量系统等检测手段,分析无害温热刺激下,TRPV3通道活化角质形成细胞释放瘙痒介质,及温热刺激下的动物行为学研究,探讨清热祛湿方是否具有调控TRPV3通道抑制角质形成细胞释放瘙痒介质的作用。期望通过本研究为中医药治疗AD的优越性提供新的理论依据。同时,对研发抗瘙痒新药提供新思路。

Pruritus is the most prominent clinical symptom of atopic dermatitis (AD), which is often aggravated by warm stimulation and at night. Controlling pruritus is an important basis for the treatment of AD. Abnormality of TRPV3 channel is an important link in the release of pruritogens from keratinocytes, leading to warmth-provoked itch. According to the theory of traditional Chinese medicine, "slight heat leads to itching". Qingre is a common method for relieving itching. Qingre Qushi recipe, which is mainly composed of Qingre herbs, can significantly improve the skin rash and itching at night in acute AD. Based on previous studies, the following scientific hypothesis was put forward: Qingre Qushi formula can regulate the release of prurituogens from keratinocytes through TRPV3 channel protein, and improve the pruritus mediated by warm heat. In this project, we constructed keratinocytes and animal models with high expression of TRPV3. Western blotting, flow cytometry, RT qPCR and scratching real-time quantitative system were used to analyze the itching signal released by keratinocytes through TRPV3 channel, and the animal behavior study under harmless warm heat stimulation, so as to explore whether Qingre Qushi formula can regulate TRPV3 channel and inhibit the release of itching mediators by keratinocytes. This study is expected to provide a new theoretical basis for the superiority of traditional Chinese medicine in the treatment of AD. At the same time, it provides new ideas for the research and development of new anti-pruritus drugs.
研究背景:瘙痒是特应性皮炎(AD)最突出的临床症状,具有痒觉异化的特征。热敏瞬时受体电位(TRPV3)通道异常是介导角质形成细胞释放瘙痒介质导致热觉痒化的重要环节。清热祛湿方前期研究显示具有显著的抗炎和免疫调节作用,前期研究表明其对AD具有良好的治疗效果,但治疗热觉痒化的具体机制尚需进一步阐明。.主要研究内容:.1. 模型建立与机制研究: 建立了MC-903诱导的AD小鼠模型、Carvacrol诱导的TRPV3高表达AD小鼠模型以及TNF-α/IFN-γ诱导的AD细胞模型,并通过质谱分析技术鉴定中药液中化学成分。.2. 药效评估: 采用治疗前后皮炎评分、耳廓肿胀度、行为学、免疫组织学染色及分子生物学等方法系统评价清热祛湿方的治疗效果。研究内容包括TRPV3高表达角质形成细胞中mRNA和蛋白表达作用,以及对AD小鼠模型的行为学影响。.3. 机制探讨: 研究了清热祛湿方对TRPV3高表达的细胞模型及AD小鼠皮肤组织病理症状、炎症因子表达及免疫调控的作用机制。.重要结果:.1. 疗效: 清热祛湿方可显著降低AD模型小鼠的皮炎评分(P<0.05),改善耳廓肿胀程度,且呈剂量依赖性。与醋酸泼尼松组相比,清热祛湿方同样具有显著的抗炎作用。对照组皮肤结构正常;模型组出现表皮增生、棘细胞层增厚、血管扩张及炎症细胞浸润等典型AD病变。用药后上述症状明显改善,TRPV3表达水平显著下调。.2. 机制研究:清热祛湿方抑制在温热刺激下AD小鼠及细胞模型中TRPV3 mRNA、蛋白表达以及分泌瘙痒相关细胞因子及介质的表达,但在TRPV3高表达的动物及细胞模型中治疗作用消失。.科学意义:.本研究首次系统揭示了清热祛湿方通过调控TRPV3通道治疗AD痒觉异化的分子机制,为中医药在炎症性皮肤病中的应用提供了理论依据。项目不仅验证了中药复方的临床疗效,还阐明了其作用靶点和信号通路,具有重要的学术价值和潜在的应用前景。
