青少年血压偏高可影响成人心血管疾病的发生发展;不良体格发育的早期干预有望能促进青少年血压健康,为降低成人心血管疾病负担提供新的思路,从而实现心血管病的预防关口前移,但目前仍缺乏婴幼儿期的队列研究证据支持。本项目基于一项自孕期开始随访长达14年的出生队列,拟研究:(1)联合应用基于组轨迹模型、分式多项式混合效应、相对增长量等轨迹分析前沿技术,全面地刻画人群及个体从出生到青少年身长/身高、BMI等体格指标的发育变化,特别是0-2岁及2岁后发育的延续性;(2)明确上述不同年龄体格发育动态与青少年血压的关联强度,及其与孕期、出生结局、性发育时相等关键变量的作用网络;(3)借助于孟德尔随机化设计,验证上述关联的因果效应。本项目的完成,可阐明0-2岁及2岁后体格发育变化对青少年血压效应强度的差异,明确早期干预的敏感窗口期和目标人群,多角度、多层次地为促进人群全生命周期心血管健康提供科学依据。
英文摘要
The blood pressure of adolescents affects the occurrence and development of the cardiovascular disease. Early-life intervention for poor physical growth is expected to improve adolescent blood pressure health and provides new thoughts for reducing the burden of cardiovascular disease during adulthood, consequently achieving the advancement of cardiovascular disease prevention. However, this hypothesis still needs scientific evidence based on cohort studies among the infant and young child population. This project takes advantage of a birth cohort that has been followed for 14 years since pregnancy and proposes the following aims. 1) Jointly apply cutting-edge techniques of trajectory analysis such as group-based trajectory modeling, mixed effect models with fractional polynomial functions, and relative growth measures to comprehensively describe the dynamic changes of physical growth (i.e., length/height and BMI) from birth to adolescence within the population and individuals, and especially the continuity of growth between 0-2 and after 2 years old. 2) Clarify the associations between dynamic changes of physical growth at different ages and adolescent blood pressure, and its net-relationships with pregnancy exposures, birth outcomes, and puberty. 3) Confirm the causal associations using the Mendelian randomization design. The promising results would quantify the strength of causal associations of dynamic growth during 0-2 and after 2 years old with adolescent blood pressure, identify the critical window and key population, and finally promote the cardiovascular health of the population from multiple lenses and a life-course perspective.
心血管病给我国乃至全球造成了严重的疾病负担。儿童队列研究表明青少年血压存在“轨迹现象”,可影响成人心血管疾病的发生发展。生命早期的体格发育与青少年血压可能存在因果关联,因此不良体格发育的早期干预有望能促进青少年血压健康,为降低成人心血管疾病负担提供新的思路,从而实现心血管病的预防关口前移。本项目搭建了一个含遗传数据等组学信息,自孕期-婴幼儿期-学龄期-青春期全景暴露信息的生命历程血压健康队列平台;该项目的科学发现主要包括:①识别了0-2岁婴幼儿体重、身长、BMI、体重身长比等指标在人群中的发育轨迹,并刻画了个体在不同年龄点的发育速率。结合人群队列和孟德尔随机化设计,明确了不同体格指标发育轨迹及速率与青少年血压的因果效应强度。②经反事实框架下的因果中介效应分析,发现青少年现时BMI是2岁内的体重发育轨迹与青少年收缩压关联的一个因果中介因素,中介比例可达到85%,但未发现对青少年舒张压也存在此效应,提示在错过关键窗口期后对肥胖进行干预可能并不能很好地逆转其对远期舒张压的危害。③将婴幼儿0-2岁划分成四个年龄段即0-6、6-12、12-18和18-24月龄,采用四种估计方法均较为一致地发现6-12月龄内的体重和身长发育增量与青少年收缩压、舒张压、脉搏压和平均动脉压存在较为一致地正向关联,提示6-12月龄可能是体格发育相对于青少年血压健康的一个敏感窗口期。④分别采用以先验知识驱动的结构方程模型和以数据驱动的动态贝叶斯网络模型,构建了青少年血压的因果作用路径,明确了生命历程各因素对青少年血压的效应大小,及时序因果网络。这些研究结果有助于在生命早期1000天识别远期心血管病“高危”群体,为实现心血管病预防关口前移提供新思路,多角度、多层次地为促进人群全生命周期心血管健康提供科学依据;对于指导本研究地区及其它发展中国家基层妇幼保健的卫生管理策略制定具有重要的公共卫生意义。
