生殖健康事关国计民生。目前育龄人口生育力下降,保障生殖健康已成为我国公共卫生的重大挑战。近年来膳食因素对生殖健康的影响备受关注,但膳食多不饱和脂肪酸(PUFAs)对生殖健康的影响尚不明确。尚缺乏PUFAs对育龄女性生殖健康、不孕女性生育力与辅助生殖技术(ART)治疗效果影响的人群流行病学及机制研究证据。本课题拟基于寻求ART治疗的女性人群开展跟踪随访。调查膳食PUFAs摄入,采用气相色谱法测定血清、卵泡液中PUFAs水平,综合评价PUFAs对基础性激素、卵巢保留功能、临床妊娠率、活产率等效应指标的影响,明确PUFAs摄入与代谢对女性生殖内分泌水平及ART疗效的影响。在此基础上结合靶向代谢组学技术,探明PUFAs影响女性卵巢功能的关键分子通路。项目预期在“暴露-反应”关系基础上为制定女性生殖健康膳食模式提供新线索,为通过PUFA膳食调节预防不孕不育、提高助孕成功率提供新的科学依据。

Reproductive health is crucial to national welfare and the people’s livelihood. Along with the current fecundity declining, it has become a significant challenge for public health to protect reproductive health. Recently, the influence of dietary factors on reproductive health has attracted much attention, however, the effects of dietary polyunsaturated fatty acids (PUFAs) intake remains unclear. Population-base epidemiological studies or mechanism studies are limited referring to the effects of PUFAs on female reproductive health and the efficacy of assisted reproduction technology (ART). This project is a prospective cohort study based on women under artificial assisted reproduction (ART) therapy. Baseline dietary intake of PUFAs will be measured by food frequency questionnaires, the levels of PUFAs in serum and follicle fluid will be measured by gas chromatography. Outcomes include basal hormone levels, oocyte quality, rate of good embryo, blastocyst formation rate, clinical pregnancy, pregnancy loss, live birth, etc. The associations between these outcome indicators and levels of PUFAs will be analyzed to explore the effects of PUFAs on reproductive health and ART outcomes. Furthermore, targeted lipidomic analysis method will be used to depict the metabolism of PUFAs in serum and follicle fluid and the roles of different metabolic pathways of PUFAs related to female ovarian function will be explored. This project will disclose the association between dietary PUFAs and reproductive health among Chinese women based on “exposure-response relationship” and provide evidence for reproduction protective diet, for protection against infertility through dietary PUFAs and for improvement of the outcomes of ART.
生殖健康事关国计民生。目前育龄人口生育力下降,保障生殖健康已成为我国公共卫生的重大挑战。近年来膳食因素对生殖健康的影响备受关注,但膳食多不饱和脂肪酸(PUFAs)对生殖健康的影响尚不明确。尚缺乏PUFAs对育龄女性生殖健康、不孕女性生育力与辅助生殖技术(ART)治疗效果影响的人群流行病学及机制研究证据。本研究建立了不孕女性队列,针对膳食习惯进行调查,并对接受IVF-ET治疗的结局进行随访,分析了涉及PUFAs相关的膳食组分对女性基础性激素水平、卵巢储备、IVF-ET治疗效果的治疗等,但本研究中未发现膳食习惯与女性生育力之间的显著关联。针对卵巢储备功能不同的人群,构建病例对照研究并进行非靶向脂质代谢组学分析,共发现441种差异代谢物,与卵巢储备正常女性相比,卵巢储备下降女性的卵泡液脂质代谢物中有205种上调,236种下调;利用差异代谢物构建卵巢储备功能的判别模型,阳离子、阴离子模式下代谢物差异绘制出的ROC对应AUC分别为0.686和0.665,对于采用代谢物对卵巢储备进行判别具有一定参考意义。
