盆底疼痛持续和反复发作是慢性前列腺炎/慢性盆腔疼痛综合征的主要症状,然而疼痛慢性化机制缺乏深入研究。我们前期工作发现盆底疼痛持续反复的前列腺炎患者存在中枢系统功能和结构的重塑,且中枢可塑性机制可能参与前列腺炎疼痛慢性化。本项目将研究(1)前列腺炎慢性疼痛调控中枢可塑性和中枢糖皮质激素受体(GR)的机制;(2)GR信号通路介导中枢可塑性调控前列腺炎疼痛慢性化的机制;(3)运动治疗对中枢可塑性和中枢GR的再平衡以及GR介导中枢可塑性对运动干预前列腺炎慢性疼痛的调控机制。这些研究对分析前列腺炎慢性疼痛调控中枢可塑性机制,揭示GR介导中枢可塑性调控前列腺炎疼痛慢性化机制以及前列腺炎运动治疗中枢调控机制具有重要的意义。

Persistent and repeated pelvic pain is the main symptom of chronic prostatitis / chronic pelvic pain syndrome, while the mechanism of chronic pelvic pain is lack of deep research. Our previous work has found that the function and structure of the central system would be remolded in patients with persistent and repeated pelvic pain, and the central plasticity mechanism might be involved in the chronicity of pelvic pain in patients. This project will study (1) the mechanism of chronic pelvic pain regulating central plasticity and central glucocorticoid receptor (GR); (2) the mechanism of GR signaling pathway mediated central plasticity regulating chronicity of pelvic pain; (3) the rebalancing of central plasticity and GR by exercise therapy and the mechanism of GR mediated central plasticity in exercise therapy for chronic pelvic pain. These studies are of great significance to analyze the mechanism of chronic pelvic pain regulating central plasticity and to reveal the mechanism of GR mediated central plasticity regulating chronicity of pelvic pain and the mechanism of central regulation in exercise therapy for chronic pelvic pain in prostatitis patients.
本课题研究发现,与健康受试者相比,CP/CPPS患者的脑功能磁共振表现出大脑结构功能的改变。fMRI BOLD序列结果显示CP/CPPS病人中下丘脑存在异常功能激活。DTI序列结果显示CP/CPPS病人中下丘脑存在异常的白质纤维束结构。通过对小鼠CP/CPPS模型进行进一步的病毒逆向神经示踪,课题组发现下丘脑室旁核(paraventricular nucleus,PVN)与前列腺存在直接的神经纤维连接,提示PVN参与前列腺的中枢神经调控和慢性疼痛进展,从而将CP/CPPS患者的中枢改变与慢性疼痛直接关联了起来。同时,课题组发现,与健康对照组相比,建模的小鼠CP/CPPS模型表现出明显的慢性疼痛表型,具体表现为:影像学上,小鼠CP/CPPS模型的前列腺超声表现出慢性疼痛改变;组织学上,小鼠CP/CPPS模型的前列腺石蜡切片表现出慢性炎症病理特征;行为学上,小鼠CP/CPPS模型表现出显著的机械疼痛超敏、焦虑和抑郁表型。总结来说,CP/CPPS患者的持续性疼痛通过GR诱导了PVN的功能和结构的重塑;重塑的PVN又反过来促进了CP/CPPS患者疼痛的慢性化进程。PVN---前列腺神经内分泌轴可能在CP/CPPS的发病中发挥重要的角色。CP/CPPS 发病机制的中枢调控因素目前鲜有报道,本项目是对CP/CPPS病因机制的新角度分析,具有重要的临床意义;同时,本项目的开展有助于进一步明确 CP/CPPS的病理生理机制,为后续CP/CPPS的治疗开辟了新的方向,提供坚实的理论基础,具有很好的临床转化前景。
