糖尿病足创面愈合缓慢、截肢率高,是创面修复领域面临的重大难题之一。目前已证实,Wharton’s Jelly间充质干细胞外泌体(WJMSC-Exos)对普通创面愈合有促进作用,但其下游作用通路尚不明确,且生理状态下不能有效富集到创面部位而影响作用效果。基于前期研究基础,我们提出将WJMSC-Exos负载于新型纤维水凝胶支架治疗糖尿病足创面的设想。为证实以上设想,本课题拟从超微、分子、细胞及动物水平研究WJMSC-Exos对糖尿病足创面的修复作用及内在机制;同时,构建新型纤维水凝胶支架负载WJMSC-Exos以达到最佳治疗浓度,并具备缓释效果,通过构建糖尿病足创面动物模型,评估负载WJMSC-Exos的新型纤维水凝胶支架对糖尿病足创面的治疗效果并探讨其可能的作用机制。本研究的开展,预期可用于糖尿病足及其他溃疡性缺损创面的治疗,为慢性难愈性创面的修复提供新的理论和研究思路。
英文摘要
Because of slow healing and high amputation rate, diabetic foot wound is one of the major problems in the field of wound repair. It has been confirmed that Wharton's Kelly mesenchymal stem cell exosomes (WJMSC-Exos)can promote the healing of wound, but its downstream mechanism is not clear, and it can not effectively enrich to the wound site in physiological state, which weaken the effect. Based on previous studies, we proposed the idea of applying WJMSC-Exos to new fiber hydrogel scaffolds for the treatment of diabetic foot wound. In order to confirm the above assumption, this project intends to study the repair effect and internal mechanism of WJMSC-Exos on diabetic foot wound from the micro, molecular, cellular and animal levels. And a novel fibrous hydrogel scaffold was constructed to load WJMSC-Exos, which means to achieve the best therapeutic concentration and sustained release effect. By constructing the diabetic foot wound animal models, the therapeutic effect of the novel fibrous hydrogel scaffold loaded with WJMSC-Exos on the diabetic foot wound was evaluated and the possible mechanism was explored. This project is expected to be useful in the treatment of diabetic foot and other ulcerative wound, and contribute new theoretical basis and research ideas for the repair of chronic refractory wound.
