Myocardial ischemia/reperfusion injury (MIRI) can lead to serious adverse cardiovascular events. Exploring effective therapeutic measures has become an urgent clinical demand. Inducing cardiac immune microenvironmental homeostasis is an important method to improve MIRI. As a non-invasive physical therapy, low intensity pulsed ultrasound (LIPUS) is confirmed to regulate immune responses and improve cardiac function. But it is unknown whether it is effective for the treatment of MIRI. Based on the result that LIPUS could reduce infarct size in MIRI mice, the applicant innovatively suggested that LIPUS irradiation might be a non-invasive therapy strategy to improve MIRI. Our previous studies have found that the unique mechanical effect of LIPUS could induce the production of endothelial exosomal MAT2A and promote intercellular communications. Moreover, we identified regulatory T-cells (Treg) as a key subset of LIPUS immune regulation. Therefore, we proposed that LIPUS could regulate Treg immune functions to restore cardiac immune homeostasis and alleviate MIRI via the transfer of endothelial exosomal MAT2a. This project will clearly optimize the therapeutic effect of LIPUS on MIRI, and reveal its molecular mechanism from the “epigenetics-metabolism-immunity” level. The aim of this project is to provide a theoretical basis for the establishment of non-drug, non-invasive myocardial protection strategies and help to solve the clinical treatment difficulties of MIRI.
我国心肌梗死(AMI)呈快速上升态势且死亡率居高不下,造成了沉重的社会与经济负担,已成为亟待解决的重大公共卫生问题。血运重建是AMI治疗的首要措施,然而随之带来不可避免的心肌缺血再灌注损伤(MIRI)。MIRI可高达最终心肌梗死面积的50%,但临床上仍缺乏明确有效的治疗措施。申请人研究证实,低强度脉冲超声(LIPUS)辐照可以减轻MIRI,包括:提高左室心功能、降低梗死面积与心肌细胞凋亡。进一步发现,LIPUS可以通过机械振动诱导细胞外囊泡(EVs)的产生,进一步通过细胞间通讯调控CD4+T细胞免疫代谢功能,减轻心脏免疫损伤。而应用GW4869阻断细胞外囊泡的产生则消除了LIPUS的治疗作用。同时,注射LIPUS诱导的EVs可以调节CD4+T细胞的免疫代谢状态,进而减轻心脏免疫损伤。以上结果证实,LIPUS可以通过机械刺激诱导EVs减轻MIRI及心脏免疫损伤,揭示了LIPUS作为一种无创物理治疗方式,在心血管疾病中的创新治疗价值。
