移植排斥反应是肝移植术后影响患者预后和生存率的关键因素,免疫抑制是主要防治手段。目前临床上治疗方案存在特异性差和因长期应用致过度抑制引起严重不良反应的缺点,迫切需要寻找特异性好且能诱导机体免疫耐受的潜在药物。促使枯否细胞向M2免疫抑制型极化在肝移植术后诱导免疫耐受中起重要作用。前期我们研究发现醋柴胡多糖具有肝靶向性和免疫抑制作用,及类纳米粒的体内过程,推测其能靶向枯否细胞诱导免疫耐受从而抑制肝移植排斥。本课题通过色谱分离技术和动物模型结合筛选抑制肝移植排斥反应的醋柴胡活性多糖,利用流式、western blot和PCR等技术研究活性多糖靶向枯否细胞使其极化从而诱导免疫耐受的作用,并基于多糖受体效应信号网,利用磁珠分选、基因过表达/沉默动物等手段从细胞和动物层面探讨其作用机制。本课题的实施为醋柴胡多糖作为肝移植术后免疫抑制剂的开发提供科学依据,为寻找新的肝移植排斥治疗策略和靶点提供分子依据。

Transplant rejection is a key factor affecting the prognosis and survival rate of patients after liver transplantation, and immunosuppression is the main means of prevention and treatment. At present, the clinical treatments have the disadvantages of poor specificity and adverse reactions caused by excessive suppression. There is an urgent need to find potential drugs with good specificity and can induce immune tolerance. Promoting Kupffer cells to M2 immunosuppressive polarization plays an important role in inducing immune tolerance after liver transplantation. In the previous study, we found that polysccharides of vinegar-baked Radix Bupleuri have liver targeting and immunosuppressive effects, as well as the nanoparticles-like process in vivo, which speculated that it can target kupffer cells to induce immune tolerance and then inhibit liver transplantation rejection. In this study, the active polysccharides of vinegar-baked Radix Bupleuri inhibiting liver transplantation rejection will be screened by chromatographic separation technique and animal model. The effect of active polysaccharides on polarizing kupffer cells and then inducing immune tolerance will be studied by flow cytometry, western blot and PCR. Based on the effect signal network of polysaccharide receptor in kupffer cells, the mechanism will be discussed on the cellular and animal level of studies by means of magnetic bead sorting, gene overexpression / silencing animals. The implementation of this project will provide a scientific basis for the development of polysccharides of vinegar-baked Radix Bupleuri as immunosuppressants after liver transplantation, and also provide molecular basis for finding new strategies and targets for the treatment of liver transplantation rejection.
肝移植术后免疫排斥反应是影响患者预后的关键因素,现有免疫抑制剂存在特异性低、长期使用导致过度抑制等临床局限性。本研究基于枯否细胞(KCs)M2型极化在诱导免疫耐受中的作用,结合前期发现的醋柴胡多糖(VBCP)具有肝靶向性和免疫抑制作用,及类纳米粒的体内过程,系统探究其通过调控KCs极化抑制肝移植排斥的活性物质基础及机制。本项目采用不同提取方法得到了5种不同提取方法多糖,对其进行了理化性质表征,通过药效筛选确定碱提醋柴胡多糖(KVBCP)得率与活性最优。进一步膜分离获得5个碱提醋柴胡多糖部位,并采用体外免疫亢进T,B细胞模型、M1型枯否细胞模型结合体内皮肤移植急性排斥小鼠模型和肝移植排斥大鼠模型筛选出了靶向枯否细胞抗肝移植排斥反应的醋柴胡多糖活性部位KVBCP3,进一步对活性部位进行活性导向分离纯化,筛选出了活性均一多糖成分KVBCP3-3,并明确KVBCP3-3为果胶类三螺旋多糖,其主链以1,4-α-GalpA为主,侧链含阿拉伯糖、半乳糖等特征结构。在机制研究中,通过构建LPS诱导的M1型KCs模型,发现KVBCP3可显著抑制M1型细胞因子水平,抑制M2型细胞因子水平,并抑制KCs M1型极化,诱导M2型极化。进一步采用靶点钩钓技术、Pull-down及质谱鉴定实验揭示KVBCP3直接作用于枯否细胞的TLR2、Dectin-1及ATP5A蛋白,采用western blot法并结合相应激动剂/抑制剂进行了体外靶点验证,然后基于体外筛选出的靶点,采用靶点“钩钓”技术结合Pull-down实验、转录组实验及激动剂/抑制剂进行了体内靶点和机制研究及验证,阐明了醋柴胡活性多糖靶向枯否细胞的TLR2、Dectin-1、ATP5A蛋白,其通过抑制枯否细胞的TLR2/MyD88/C-JUN、TLR2/MyD88/NF-κB,Dectin-1/NF-κB和TLR2-Dcn信号通路从而抑制枯否细胞M1型极化,诱导M2型极化进而诱导免疫耐受发挥抗肝移植排斥反应的机制。本项目阐明了醋柴胡多糖通过靶向KCs极化调控肝移植免疫耐受的多维机制,为开发肝靶向天然免疫抑制剂提供了新策略,研究成果兼具原创性与转化潜力,为肝移植术后精准免疫治疗提供了新思路。
