结直肠癌肿瘤微环境中CD8+T细胞的浸润数量普遍较少,是引发肿瘤免疫逃逸的主要原因。肠道菌群失衡与肿瘤免疫微生态的紊乱密切相关,但其机制仍不清楚。我们前期研究发现,肠道菌群来源的内生性乙醇,其产量与肿瘤微环境中CD8+T细胞的浸润数量呈高度负相关,且能导致趋化因子转录调控蛋白RUNX3失活,抑制肿瘤细胞表达CD8+T细胞相关趋化因子CXCL9。本项目将结合微生物学、分子病理学及分子生物学研究方法,通过体内外实验,首次揭示肠道菌群通过释放乙醇抑制CD8+T细胞浸润,促进结直肠癌免疫逃逸的作用与机制。基于肿瘤微环境中CD8+T细胞的浸润数量与患者PD-1治疗敏感性密切相关,我们还将探明内生性乙醇对结直肠癌PD-1治疗敏感性的影响作用。本研究不仅为进一步阐明肠道菌群失衡促进肿瘤免疫逃逸的分子机制提供可靠的证据,还为确立内生性乙醇作为改善结直肠癌患者PD-1治疗敏感性的干预靶点提供充分的科学依据。

The infiltration of CD8+T cells in the tumor microenvironment of colorectal cancer is generally low, which is the main cause of tumor immune escape. The imbalance of intestinal bacteria is closely related to the disorder of tumor immune microecology, but its mechanism is still not clear. Our previous studies have found that the output of intestinal bacteria derived endogenous ethanol is highly negatively correlated with the number of CD8+ T cells infiltrated in the tumor microenvironment, and can cause the inactivation of the chemokine transcriptional regulatory protein RUNX3, as well as inhibiting the expression of CD8+ T cell related chemokine CXCL9 in tumor cells. This project intends to explore the role and mechanism of the intestinal bacteria derived endogenous ethanol in reshaping the tumor immune microecology, which promote the immune escape of colorectal cancer, by using microbiology, molecular pathology and molecular biology research methods in vivo and in vitro experiments. Because the infiltration of CD8+T cells in the tumor microenvironment is also closely associated with the sensitivity of CRC patient to PD-1 treatment, we will also explore the effect of endogenous ethanol on the sensitivity of CRC patients to PD-1 treatment. This project not only provides a new scientific basis for elucidating the molecular mechanism of intestinal bacteria in promoting tumor immune escape, but also initially clarifies the possibility of intervention in endogenous ethanol production as an effective measure to improve the sensitivity of PD-1 treatment in CRC patients.
1.研究背景. 免疫逃逸是导致结直肠癌演进及患者不良预后的重要原因,如何遏制其发生仍为世界性难题。现有研究已明确,肿瘤微环境中CD8+T细胞浸润数量的普遍较少,不仅是引发结直肠癌免疫逃逸的主要原因,还是导致肠癌患者PD-1治疗敏感性较低的重要因素。而肿瘤细胞表面抗原呈递减少则是抑制肿瘤微环境招募CD8+T细胞的关键因素。因此,提高肿瘤细胞免疫原性是促进肿瘤微环境招募CD8+T细胞,阻止结直肠癌免疫逃逸发生,提高肿瘤患者PD-1治疗敏感性的关键策略。然而,目前关于肿瘤细胞表面抗原呈递异常的发生机制仍不清楚,亟待进一步研究阐明。..2.研究内容. 本项目研究分为三部分:首先,我们将通过临床样本检测,明确结直肠癌肿瘤细胞ADH1A表达水平与结直肠癌患者预后及肿瘤微环境CD8+T细胞浸润数量的关系;进而,我们将通过体内、外功能实验,明确ADH1A通过促进肿瘤细胞HLA-1表达,增强肿瘤微环境招募CD8+T细胞的能力,从而提高肿瘤细胞对PD-1治疗敏感性的作用;最后,我们还将通过GO、GSEA分析、临床样本及体内外实验样本验证和相关恢复实验,明确JAK/STAT信号通路在ADH1A调控结直肠癌细胞HLA-1表达过程中的重要性。..3.关键研究结果及意义 . 本课题组首次发现结直肠癌细胞乙醇代谢酶ADH1A的异常表达与肿瘤细胞MHC-1的表达水平及肿瘤微环境CD8+T细胞的浸润数量密切相关。目前仍无研究揭示乙醇脱氢酶体系异常表达对结直肠癌演进及肿瘤免疫微环境异常的调控作用及分子机制。本项目聚焦肿瘤细胞乙醇代谢酶体系,首次探明结直肠癌细胞ADH1A表达的下调通过减少细胞表面抗原呈递,抑制肿瘤微环境招募CD8+T细胞的作用与机制。同时,我们还探明高表达ADH1A对肿瘤细胞PD-1治疗敏感性的增强作用,不仅为进一步阐明结直肠癌细胞免疫逃逸的分子机制提供有力的证据,还为防治结直肠癌演进、提高肿瘤患者PD-1治疗敏感性提供有效的干预靶点。
