在免疫治疗彻底改变非小细胞肺癌治疗格局的同时,仍有大量患者无法获得理想的治疗效果,其中,多项研究表明STK11突变与非小细胞肺癌免疫治疗耐药相关。本项目研究证实,携带STK11突变的细胞株对多种MDM2抑制剂敏感,其可以通过稳定细胞内p53蛋白浓度,诱导肿瘤细胞发生凋亡及铁死亡双重程序性死亡事件,经由MDM2-P53中枢分别从HIF1-mTOR及pSTAT3-SLC7A11相关信号通路,逆转STK11突变导致的凋亡抵抗及肿瘤细胞对还原态底物的依赖。此外,在免疫浸润微环境方面,P53可以通过抑制pSTAT-IL6途径改善STK11突变导致的肿瘤相关中性粒细胞浸润水平,逆转其对免疫治疗的耐药抵抗。本研究围绕STK11耐药抵抗相关机制及探讨潜在治疗方案为相关患者提供针对性治疗方案。

While immunotherapy has revolutionized the treatment landscape of non-small cell lung cancer, there are still a large number of patients who are unable to achieve the desired therapeutic outcome. Among them, several studies have shown that STK11 mutations are associated with immunotherapy resistance in non-small cell lung cancer. In this project, we demonstrated that cell lines carrying STK11 mutations are sensitive to various MDM2 inhibitors, which can induce dual programmed death events of apoptosis and iron death in tumor cells by stabilizing intracellular p53 protein concentrations, and reversing STK11 resistance and iron death through the HIF1-mTOR and pSTAT3-SLC7A11-related signaling pathways via the MDM2-P53 hub, respectively. mutations resulted in apoptosis resistance and tumor cell dependence on reduced state substrates. In addition, in the immune infiltration microenvironment, P53 could improve the level of tumor-associated neutrophil infiltration caused by STK11 mutations and reverse their resistance to immunotherapy by inhibiting the pSTAT-IL6 pathway. This study focuses on the mechanisms related to STK11 drug resistance and explores potential therapeutic options to provide targeted treatment options for the patients concerned.
约15-30%的NSCLC患者携带STK11/LKB1突变,此类突变通过干扰肿瘤免疫识别功能及多种治疗通路,导致免疫治疗耐药,甚至无药可用。本项目以STK11/LKB1突变肺癌为核心,开展了药物敏感性、分子机制及治疗策略的系统研究。研究发现,STK11/LKB1突变通过异常调控氧化磷酸化及STAT3通路,影响细胞凋亡和代谢稳态。公开数据库和体外实验验证了STK11/LKB1突变细胞对MDM2抑制剂的高敏感性。进一步构建突变及过表达细胞系,揭示MDM2抑制剂通过诱导pSTAT3下调、激活Bcl-2/MCL-1依赖性凋亡的机制。此外,STK11/LKB1突变引发的mTOR-AMPK-HIF-1α异常通路,可通过p53稳定性调控SLC7A11表达,从而增强铁死亡效应。在PDX/CDX模型中,MDM2抑制剂APG-115表现出显著的抗肿瘤活性,验证了其临床应用潜力。.在EGFR-TKIs耐药后的小细胞肺癌转化(T-SCLC)研究中,通过多组学分析,首次发现T-SCLC的分子特征介于肺腺癌(LUAD)与原发性小细胞肺癌(SCLC)之间,并进一步将其分为LUAD样和SCLC样亚型。匹配治疗策略的应用显著延长了患者的无进展生存期(PFS),为精准治疗提供了理论基础。此外,通过回顾性研究评估了阿替利珠单抗联合化疗在EGFR突变T-SCLC患者中的疗效,发现免疫治疗显著延长了总生存期(20.2个月 vs 7.9个月),并通过分子特征分析探讨了治疗获益和耐药机制。.研究揭示了STK11/LKB1突变及T-SCLC的独特分子机制和治疗策略,推进了精准医学在肺癌治疗中的应用。.项目成果包括1篇SCI论文发表及2篇在投返修,获得国家重大专项资助1项,横向课题 1 项,专家共识1项。
