血脑屏障的存在和胶质瘤的高度异质性、抑制性免疫微环境和高重塑性是脑胶质瘤免疫治疗响应率低的主要原因。巨噬细胞/小胶质细胞占胶质瘤浸润免疫细胞数量的50%以上,是改善肿瘤免疫微环境的重要靶标。浸润的巨噬细胞主要为促癌的M2型巨噬细胞,因此重编程巨噬细胞/小胶质细胞可有效改善肿瘤抑制微环境,但靶向重编程巨噬细胞/小胶质细胞药物的缺乏和血脑屏障阻碍了脑胶质瘤免疫微环境的改善。本课题前期通过抑制USP7去泛素化酶活性可有效调控MAPK信号通路,促进M2型巨噬细胞重编程,并且前期研究揭示肿瘤细胞放疗后微囊泡可作为天然药物载体靶向巨噬细胞,但无法有效穿透血脑屏障;基于此,本课题以基因编程手段使包裹USP7抑制剂的放疗后微泡膜表面表达转铁蛋白和SR-B1靶向性多肽R4F,促进微泡浸润大脑,靶向调控巨噬细胞/小胶质细胞的MAPK信号通路,增强PD-1抗体的免疫应答。该研究将为临床治疗脑胶质瘤患者提供新思路。

The existence of blood-brain barrier and the high heterogeneity, inhibitory immune microenvironment and high remodeling of glioma are the main reasons for the low response rate of glioma immunotherapy.Macrophages/microglia cells account for more than 50% of the number of infiltrated immune cells in glioma, and are important targets for improving the tumor immune microenvironment.The infiltrating macrophages are mainly M2-type macrophages that promote cancer. Therefore, reprogramming macrophages/microglias can effectively improve the tumor inhibition microenvironment. However, the lack of drugs targeting reprogramming macrophages/microglias and the blood-brain barrier hinders the improvement of the immune microenvironment of glioma.In the early stage of this study, MAPK signaling pathway can be effectively regulated by inhibiting USP7 deubiquitination enzyme activity to promote the reprogramming of M2-type macrophages. In addition, previous studies revealed that microvesicles of tumor cells after radiotherapy can be used as natural drug carriers to target macrophages, but they cannot effectively penetrate the blood-brain barrier.Based on this, this study used genetic programming to make the surface of the radiotherapy microvesicle coated with USP7 inhibitor express transferrin and SR-B1 targeting polypeptide R4F, promote the infiltration of microvesicle into the brain, regulate the MAPK signaling pathway of macrophages/microglia, and enhance the immune response of PD-1 antibody.This study will provide a new idea for the clinical treatment of patients with glioma.
项目背景:肿瘤微环境在肿瘤的发生、发展和转移中起着关键作用,其免疫抑制特性使得许多免疫治疗策略效果受限。近年来,免疫原性细胞死亡(ICD)和铁死亡(ferroptosis)等新型细胞死亡方式受到关注,它们能够通过改变肿瘤细胞特性及免疫微环境来增强抗肿瘤免疫反应。此外,炎症体的激活等过程也与肿瘤的免疫调节密切相关。.主要研究内容:本项目主要围绕肿瘤微环境中的关键因素展开研究,包括基于微粒的肿瘤微环境调控与治疗、炎症体在肿瘤中的作用及靶向治疗、铁死亡诱导与免疫微环境重塑。研究了辐射处理后的肿瘤细胞来源微粒(RMPs)在脑转移瘤治疗中的应用,探讨了炎症体(如NLRP3、NLRC4、NLRP1和AIM2)在不同类型肿瘤中的表达模式、激活机制及其对肿瘤免疫微环境的影响,并设计了一种载有铁死亡诱导剂RSL-3和凋亡诱导肽CT20p的辐射肿瘤细胞来源微粒(RC@RMPs),研究其在前列腺癌中的抗肿瘤效果。.重要结果与关键数据:成功构建了携带USP7抑制剂的RMPs(P5091@RMPs-R4F),该微粒能够有效跨越血脑屏障,靶向脑转移瘤中的M2型巨噬细胞,显著提高小鼠的生存率,与免疫检查点阻断疗法联合使用时效果更佳。发现炎症体在不同肿瘤类型和阶段中具有不同的作用,如NLRP3炎症体在某些肿瘤中通过促进免疫细胞浸润和细胞因子分泌发挥抗肿瘤作用,而在其他肿瘤中则可能通过抑制免疫反应促进肿瘤进展。RC@RMPs能够显著诱导前列腺癌细胞的铁死亡,通过激活树突状细胞(DCs)和促进M1型巨噬细胞极化重塑肿瘤免疫微环境,显著抑制RM-1肿瘤生长,延长小鼠生存时间,与抗PD-1单抗联合使用时效果更佳。.科学意义:本项目的研究成果为肿瘤治疗提供了新的思路和策略。通过调控肿瘤微环境中的关键因素,如利用微粒载体靶向递送药物、激活或抑制炎症体以及诱导铁死亡等,有望打破肿瘤免疫抑制微环境,增强抗肿瘤免疫反应,为开发新型肿瘤免疫治疗药物和联合治疗方案提供了理论依据和实验基础。这些发现不仅丰富了肿瘤免疫治疗的理论体系,也为未来肿瘤治疗的临床应用提供了潜在的新靶点和新方法。
