双酚A替代物(BPs)具有内分泌干扰作用,随着儿童肥胖率持续上升,BPs对不同代谢型肥胖的影响和机制缺乏人群研究,严重影响肥胖的精准干预和BPs暴露的健康效应评估。本课题拟采用高效液相色谱串联质谱仪法,检测2040名儿童尿液7种BPs代谢物浓度,依据BPs暴露水平分为高、中和低暴露组,开展2年随访队列研究,分析儿童BPs暴露对不同代谢型肥胖发生和进展的影响。筛选基线和随访中代谢健康型和异常型肥胖者各100名,设立2组病例组,按1:1匹配体重正常且代谢健康和代谢异常者各100名,为2组对照组,建立前瞻性巢式病例对照研究,采用基因列阵质谱分析系统检测基线和终期部分脂代谢功能基因的甲基化水平,评估甲基化在儿童BPs暴露影响不同代谢型肥胖中的表观遗传学机制。本课题研究结果将为明确BPs暴露的健康效应和阐明肥胖的环境因素致病机制提供循证依据,为制定BPs接触限值和优化肥胖精准干预策略提供新思路。

As the childhood obesity prevalence continues to rise, there is lack of study on the effects and mechanisms of bisphenol A substitute (BPs) on childhood metabolic obese phenotypes. The precise intervention to obesity and the assessment of the health effects of population BPs exposure are severely restricted. The concentrations of 7 BPs metabolites in the urine of 2040 children are detected using the High Performance Liquid Chromatography-Tandem Mass Spectrometry. According to the BPs concentrations' levels, all the children are divided into High, Middle and Low exposure groups. A 2-year follow-up cohort study is conducted to analyze the effects of population BPs exposure on the occurrence and progression of childhood metabolic obese phenotypes. Two case groups are set by screening the 100 metabolically healthy obesity and 100 metabolically unhealthy obesity children at the base line and follow-up visits. Two matched control groups with separate 100 children are selected in children with metabolically healthy normal weight and metabolically unhealthy normal weight. A prospective nested case-control study is established by detecting lipid metabolism genes methylation level at baseline and final investigation using DNA MS array analyzer. This part design is used to evaluate the epigenetic mechanism of methylation in the effects of BPs exposure on childhood metabolic obese phenotypes. The results of this study will provide an evidence-based basis for clarifying the population health effects of BPs exposure and the pathogenesis of obesity caused by environmental factors, and provide a new idea for formulating BPs exposure limits and optimizing precise intervention strategies for obesity.
本项目旨在探究儿童期双酚A替代物(BPs)暴露对儿童不同代谢型肥胖的影响,及其通过脂代谢基因甲基化水平变化影响肥胖发生的机制。鉴于双酚A替代物的广泛使用和潜在健康风险,本项目对于理解环境内分泌干扰物(EDCs)对儿童代谢健康的影响具有重要意义。.本项目依托高通量、高灵敏度的仪器平台和分析方法进行了各类检测指标的定量。采用优化的SPE联合高效液相色谱串联质谱法,检测了2300多份儿童尿液样本中的BPs暴露水平,关注7种BPs代谢物及BPA的体积分数浓度和肌酐校正浓度,并依据暴露水平分为高、中和低暴露组,通过前瞻性巢式病例对照研究,分析BPs暴露对不同代谢型肥胖的影响;同时,项目还关注脂代谢功能基因(LOXL3和SLC6A19)的甲基化水平,以揭示EDCs暴露影响肥胖发生的表观遗传学机制,采用LC-MS/MS系统和高通量MALDI-TOF MS法,本研究检测了140多名儿童的非靶向脂代谢谱水平,并对200份儿童血液样本中脂代谢功能基因的甲基化水平进行了检测。基于上述生物学监测,本研究通过横断面研究、前瞻性队列研究和前瞻性巢式病例对照研究,探讨双酚类物质(BPs)暴露与儿童不同代谢型肥胖的关联及作用机制。研究发现厦门市儿童中,内分泌干扰物BPs和多环芳烃(PAHs)暴露普遍,且呈上升趋势,共同暴露风险高,其暴露显著增加BMI,与代谢健康型肥胖(MHO)和代谢异常型肥胖(MUO)存在显著关联,BPAP影响更强,且两者联合暴露存在协同效应。广义估计方程模型和BKMR模型结果均显示BPs和PAHs暴露与肥胖存在剂量-反应关系及线性关联。BPs和PAHs暴露具有不同脂质代谢特征,脂质代谢部分中介其与MHO和MUO关联,MUO中暴露对脂质代谢影响更复杂。MHO和MUO脂质代谢特征差异明显,涉及不同代谢通路,BPs和PAHs与显著相关代谢物及通路也存在差异。中介效应分析显示,MHO中BPs和PAHs效应主要集中在鞘脂类和固醇类代谢物,MUO中介效应更广泛。此外,对于二者关联的表观遗传学机制的相关研究结果,本研究进一步分析和论证并发现了与BPs同为EDCs的PAEs通过表观遗传学改变影响儿童肥胖结局的证据。.本研究揭示BPs的内分泌干扰及对肥胖表型的影响,为儿童BPs暴露的限值设定提供循证依据,为儿童肥胖代谢疾病早期风险的科学精准管理提供重要依据,有重要的公共卫生意义和科学意义。
