溃疡性结肠炎(UC)是引起腹痛的临床常见病之一,腹部推拿对UC具有较好的镇痛效果,但其作用机制不明。研究表明,外周敏化是UC疼痛的重要机制,背根神经节(DRG)中PI3K/Akt通路调控钠钙交换体(NCX),介导DRG神经元细胞外的钙内流,致胞内钙浓度增加,引起外周敏化。我们前期发现:腹部推拿对UC具有较好的镇痛作用,且能抑制UC大鼠DRG神经元细胞钙的内向电流、降低细胞内钙浓度水平,抑制其外周敏化。鉴于此,我们推测:腹部推拿可能通过抑制DRG中PI3K/Akt信号通路,弱化NCX表达,降低DRG神经元细胞内钙浓度,抑制其外周敏化,从而对UC起到镇痛作用。本研究拟用疼痛行为学、免疫生物学、电生理学等多学科技术,观察腹部推拿对UC大鼠DRG中PI3K/Akt通路调控NCX的作用,及对DRG神经元胞内钙浓度的影响,从而揭示腹部推拿对UC的外周镇痛机制,为其临床应用提供理论依据及奠定实验基础。

Ulcerative colitis (UC) is one of the common clinical diseases causing abdominal pain. Abdominal massage has a good analgesic effect on UC, but its mechanism is unknown. The studies are showed that peripheral sensitization is an important mechanism of UC pain. PI3K/Akt pathway in DRG regulates sodium calcium exchanger (NCX), mediating calcium influx of DRG neurons, increasing intracellular calcium concentration,to cause peripheral sensitization. In the early stage, we found that abdominal massage has a good analgesic effect on UC.and we found that abdominal massage could inhibit the inward current of calcium in DRG neurons of UC model rats, reducing the level of intracellular calcium, to inhibit its peripheral sensitization. In view of this, we speculate that abdominal massage may play an analgesic role in UC by inhibiting PI3K / Akt signaling pathway in DRG, weakening NCX expression, reducing intracellular calcium concentration of DRG neurons and inhibiting peripheral sensitization. In this study, animal pain behavior, immunobiology, electrophysiology and other multi-disciplinary techniques will be used to observe the effect of abdominal massage on the regulation of NCX by PI3K/Akt signaling pathway in DRG of UC rats and the effect of intracellular calcium concentration of DRG neurons,so as to reveal the peripheral analgesic mechanism of abdominal massage on UC and provide theoretical basis and experimental basis for its clinical application.
由于溃疡性结肠炎(Ulcerative colitis,UC)是一种以腹痛为主要症状且发病率在我国乃至全球范围内正在增加的慢性肠道疾病,对其发病机制及治疗手段的研究一直备受关注;腹部推拿治疗UC腹痛的疗效确切,但其治疗机制尚未明确。随着现代研究的深入,痛觉敏化(包括外周敏化和中枢敏化)被认为是UC疼痛发生的重要神经机制之一。近年来,PI3K/Akt通路在UC发病机制中的作用逐渐受到研究者们的关注:国内外多项研究已经证实无论是UC模型鼠还是UC患者都存在PI3K/Akt信号传导通路的激活现象;且有研究发现,抑制PI3K/Akt通路的传导能缓解大鼠肠道疼痛。又研究发现,PI3K/Akt信号通路上Akt磷酸化能增加DRG神经元Ca2+浓度。基于此,本研究以UC大鼠为模型,初步阐释腹部推拿治疗UC疼痛的作用机制,完善腹部推拿治疗UC腹痛的理论来源,为临床提供有力的科学依据。本研究采用5%葡聚糖硫酸钠溶液(DSS)自由饮用诱导UC大鼠模型,并以大鼠体质量、大便性状、粪便隐血情况等疾病活动指数评分(DAI评分)作为评价指标验证模型,采用腹部撤退反射评分、乙酸扭体反应进行行为学检测,运用HE染色观察结肠黏膜形态,邻-甲酚酞络合铜比色法检测DRG神经元Ca2+浓度,运用Western blot检测、免疫荧光法检测PI3K/Akt通路中关键蛋白PI3K、Akt、p-Akt等的表达,光学显微镜下观察脊髓背角尼氏体形态变化。研究结果显示,腹部推拿能降低UC模型大鼠DRG神经元Ca2+浓度,同时,腹部推拿可以有效抑制脊髓背角PI3K/Akt通路表达;并以此为前提,设置通路激活,进一步验证腹部推拿借助PI3K/Akt通路对UC脊髓背角神经元活性产生影响,从而改善UC大鼠腹痛的作用机制。本研究阐释了腹部推拿治疗UC腹痛的作用机制将有助于提高其临床应用水平,对于未来深化腹部推拿基础研究、促进学科发展具有一定的推动意义。
