非酒精性脂肪性肝炎(NASH)是一种慢性肝脏代谢性炎症疾病。其病理的不可逆性一直困扰着其临床诊疗的进展。深入探索NASH进展中的关键靶标具有重要的临床意义和科研价值。本研究从肝细胞对炎症微环境的起始板机作用这一角度切入,基于高通量检测,筛选NASH进展中的关键靶点。通过临床样本及NASH模型,初步发现肝细胞TMEM154可以通过介导TNFα分泌促进NASH中炎症微环境的形成。初步机制探索提示肝细胞TMEM154通过促进线粒体氧化呼吸链进而引起ROS产生及肝细胞TNFα的分泌增加。本研究拟进一步采用体内外NASH模型、联合蛋白组学检测、深入分析肝细胞TMEM154对于氧化呼吸链的精准作用靶点,并通过阻断实验阐明肝细胞TMEM154介导氧化呼吸链对NASH中炎症微环境的调控作用。本课题旨在以肝细胞TMEM154为靶标,通过诠释其在NASH中的作用及具体机制,为未来的临床防治探索提供新的思路。

Non-alcoholic steatohepatitis (NASH) is a chronic metabolic inflammatory liver disease that can develop into cirrhosis and even liver cancer. The irresability of its pathology has been troubling the progress of its clinical diagnosis and treatment. Detailed investigation of the crucial target involving in the process of NASH present important clinical significance and scientific research value. This study is based on the pointcut that the effect of hepatocyte on inflammatory micro-environment at the initial stage of NASH, and based on a high-throughput microarray detection. Based on the expression validation in patients’ samples and the NASH model in vivo and vitro, we found hepatic TMEM154 could aggravate the inflammatory micro-environment via enhancing hepatic secretion of TNFα. Preliminary mechanism analysis suggested hepatic TMEM154 promotes ROS production via enhancing mitochondrial oxidation respiratory chain (MORC), then promoting the secretion of TNFα. In this study, we proposed to use the NASH model in vivo and vitro, and the proteomics detection and analysis, to deeply analyze the precise target of hepatic TMEM154 for MORC, and use joint blockers and truncated mutation to clarify that hepatic TMEM154-mediated MORC induces the inflammatory micro-environment during NASH. This study aims to provide a theoretical basis for the future construction of multi-cell multi-dimensional framework of NASH, and for clinical prevention and treatment of NASH by interpreting the role and specific mechanism of hepatic TMEM154 in the development of NASH.
非酒精性脂肪性肝炎(NASH)是一种常见的肝脏慢性疾病,已成为肝硬化和肝癌的主要风险因素之一。近年来,NASH的发病机制和治疗策略成为肝病研究的重要课题。课题组围绕肝细胞TMEM154介导的线粒体氧化呼吸链对NASH进展中炎症微环境的调控作用及其分子机制,旨在揭示NASH及相关消化道疾病的分子病理机制,并为新型治疗靶点的开发提供理论依据。.首先,课题组探索肝细胞TMEM154在NASH中的功能及机制,发现TMEM154通过介导肝细胞线粒体自噬缺失,进而导致线粒体氧化呼吸链的功能异常,增加ROS(反应性氧种)产生,促进TNFα的分泌,形成促炎症的微环境,推动NASH的进展;其次,研究Miz1在NASH中的抑制作用,发现Miz1通过与肝巨噬细胞分泌的TNFα形成正反馈环路,推动NASH进展。此外,本项目还涉及消化道恶性肿瘤研究,特别是结直肠癌(CRC)的发生机制,揭示了GPR176与GNAS复合体通过cAMP/PKA/BNIP3L信号通路抑制线粒体自噬,促进CRC进展。课题组研究采用了小鼠模型、临床样本和细胞实验等多种方法,验证了上述机制的存在与作用。在NASH的动物模型中,肝细胞特异性敲除TMEM154的小鼠(TMEM154ΔHep小鼠)表现出NASH进展的显著减缓,肝脏的脂质沉积和纤维化程度明显降低,进一步证实了TMEM154在NASH中的关键作用。通过体外实验,发现TMEM154过表达可以抑制线粒体自噬,进一步加重NASH进展,而其缺失则能促进线粒体自噬,有效减缓疾病进展。在消化道肿瘤研究方面,本项目首次揭示了GPR176在结直肠癌中的异常表达及其促进肿瘤进展的作用。具体机制为GPR176通过与GNAS形成复合体,激活cAMP/PKA/BNIP3L信号通路,抑制线粒体自噬,从而加剧CRC的恶性进展。这一发现为CRC的早期诊断和治疗提供了新的思路。总之,本项目通过揭示线粒体自噬在消化道慢性疾病及肿瘤中的潜在机制,拓宽了研究视野,并为疾病的临床干预和治疗提供了新的理论基础。研究成果具有重要的科学意义,为相关疾病的早期诊断、靶向治疗和个性化治疗策略的制定提供了新的方向。
