Ovarian hyperstimulation syndrome (OHSS) is a serious complication during in vitro fertilization (IVF) procedure. The overexpressed VEGF, ANGPT1, and/or SPRY2 are suggested to be major factors that contribute to the pathogenesis of OHSS. To date, there is no efficient method for the early prevention and treatment of OHSS. Mimic the hCG effect, Kisspeptin-54 has been used to trigger oocyte maturation and can reduce the development of severe OHSS. Our preliminary results showed that OHSS patients had significantly lower Kisspeptin-54 levels in the follicle fluid. In addition, treatment of granulosa cells with Kisspeptin-54 downregulated VEGF, ANGPT1 and SPRY2 expression. These results suggest that Kisspeptin-54 suppresses OHSS development by inhibiting VEGF/ANGPT1/SPRY2 expression in human granulosa cells. However, the underlying molecular mechanism remains unknown. In this research proposal, we aim to examine the effects of Kisspeptin-54 on VEGF/ANGPT1/SPRY2 expression in human granulosa cells and explore the underlying molecular mechanism in order to identify the therapeutic target for OHSS. Using OHSS rat model, Kisspeptin-54 injection will be performed to study the effect of Kisspeptin-54 on the pathogenesis of OHSS and related mechanisms. These results will provide not only important insights into the molecular mechanisms mediating Kisspeptin-54-regulated OHSS but also identify potential targets that can be used for the prevention and treatment of OHSS.
卵巢过度刺激综合征(Ovarian hyperstimulation syndrome, OHSS)是辅助生殖技术最严重的并发症,临床分子特征为血管活性因子和类固醇甾体类激素生成关键酶在OHSS患者卵巢组织高表达;截至目前,OHSS的早期预防和治疗方法极为有限。已知Kisspeptin-54可替代传统hCG扳机促进卵母细胞成熟,同时降低不孕症患者重度OHSS发生率,推测Kisspeptin-54及其上下游调控因子可能参与阻断OHSS发生,但具体调控机制不明。课题组关注Kisspeptin-54在卵巢组织的上下游调控作用,目的在于以Kisspeptin-54为核心构建分子调控网络,在此基础上揭示OHSS发生的分子调控机制,并寻求临床诊治新策略。.经过本课题组的系统性研究,发现与对照组相比,OHSS患者卵巢Kisspeptin-54及其受体KISS-1R低表达,特异性蛋白AREG、GDF-8和TGF-ß1高表达,特异性基因群VEGF、ANGPT1、SPRY2、COX-2、SPARC、CYP19A1 和StAR高表达。Kisspeptin-54在人颗粒细胞通过KISS-1R和ERK1/2信号通路抑制VEGF、SPARC、CYP19A1 和StAR 表达,此外AKT信号通路参与SPARC表达。AREG、GDF-8和TGF-ß1在人颗粒细胞抑制Kisspeptin-54(KISS-1)和KISS-1R表达。最后,项目成功建立Kisspeptin-54扳机OHSS大鼠模型,发现虽然该组卵巢重量增加,卵巢组织OHSS高表达基因群(Vegf、Sparc、Cyp19a1、Star)稍增高,但与hCG扳机OHSS组相比,卵巢重量、卵巢组织OHSS高表达基因群显著降低,说明Kisspeptin-54扳机可降低OHSS风险。.本课题首次研究Kisspeptin-54/KISS-1R在对照患者和OHSS患者的表达情况,揭示了Kisspeptin-54在卵巢组织发挥作用的上下游调控因子,构建以Kisspeptin-54为核心的分子调控网络,在此基础上系统性阐述OHSS发生的分子机制,通过OHSS大鼠模型提出临床诊治新策略。该项目的实施对分子水平上理解OHSS的发生及Kisspeptin-54在治疗OHSS方面的作用具有重要的指导意义。
