气温和大气颗粒物在时间和空间上相互交织严重危害精神健康。我国精神分裂症的疾病负担位列世界首位,但气温、PM2.5及其主要成分PM1暴露对精神分裂症的交互作用还不清楚,暴露效应的地区差异尚未探究。本项目拟在长三角地区开展研究:(1)联合使用卫星遥感反演、空间插值、地理定位等方法精准评估气温(高温/热浪、低温/寒潮、温度变异)和颗粒物(PM2.5、PM1)暴露的时空演变规律;(2)从群体和个体暴露出发,利用DLNM、logistic和分段回归模型,分析暴露因素对精神分裂症的短期影响,辨识有害暴露方式、敏感人群和交互作用;(3)应用高运算效率INLA法替代传统MCMC法下的半参数零膨胀贝叶斯时空条件自回归模型,揭示暴露因素对该疾病长期影响的局部空间差异,快速精准定位高风险地区;(4)量化暴露因素引起的疾病负担(归因风险和寿命损失年)。本项目将为长三角地区精神分裂症的预警和防治规划提供科学依据。

Spatial and temporal interaction of air temperature and ambient particulate matter seriously jeopardizes mental health. Globally, the disease burden of schizophrenia rank first in China, but little is known about the synergistic effects of exposure to air temperature, PM2.5, and its main component PM1 on schizophrenia. Meanwhile, the spatial difference in the effects of exposure to air temperature and PM2.5/PM1 on schizophrenia has not yet been investigated. This project aims to conduct research in the Yangtze River Delta, including: (1) jointly using methods of satellite remote sensing inversion, spatial interpolation and geocoding to precisely assess population exposure to air temperature (high temperature/heatwave, low temperature/cold spell, temperature variability) and particulate matter (PM2.5, PM1) as well as the underlying spatiotemporal changing patterns; (2) using DLNM, logistic, and segmented regression model to assess the short-term effects of exposures at the population and individual level on schizophrenia, aiming to identify the exposure modes of the hazards, vulnerable populations, and joint effects of the hazards; (3) applying the INLA approach with high computational efficiency instead of traditional MCMC approach to semiparametric zero-inflated Bayesian spatiotemporal conditional autoregressive model to reveal the regional differences in the long-term effects of exposures on schizophrenia with the aim to rapidly and accurately identify the high-risk area; (4) quantifying the exposure-induced disease burden including attributable risk and years of life lost. This project will provide the basis for the early-warning of schizophrenia and the development of prevention and control measures in the Yangtze River Delta.
气候变化背景下,气温和大气颗粒物在时间和空间上相互交织严重危害精神健康,但我国目前关于温度和颗粒物的独立和联合暴露对精神分裂症的短期影响及其在不同时间和空间上的特点尚不清楚。本研究利用长三角地区的高时空分辨率气象因素、空气污染物以及多种类型的精神分裂症资料构建数据库来分析多种时间尺度的温度变化(季节、天、日间、小时水平)、不同的温度暴露方式(高温、低温、超额冷/热强度、温度变异)和不同粒径的颗粒物(PM10、PM2.5、PM1)暴露对精神分裂症发作的短期影响。主要结果包括:(1)近年来,长三角地区夏季整体气温较高,在2022年出现破纪录的异常热浪事件;夜晚超额热强度整体上高于白天;(2)我国精神分裂症发作存在两个高峰期(每年3月和10月)且不同时间尺度的温度变化均与精神分裂症发作存在关联,包括每日温度、昼夜温差和日/夜间超额热以及数小时内的温度改变;(3)不同粒径颗粒物暴露均与精神分裂症发作有关,粒径越小的颗粒物(尤其是PM1)影响越大,且相对于门诊和急诊,对入院的影响更大;(4)颗粒物与温度之间存在显著交互作用,冷季中颗粒物对精神分裂症的影响更大,且颗粒物的影响在COVID-19前后有明显变化;(5)进一步丰富了该领域的方法学,包括信息流行病学(弥补因疾病或健康数据收集的滞后性而导致实时或及时的研究数据缺乏),人群类实验设计和中断时间序列分析(利用自然或人为发生的干预事件,模拟随机对照实验设计,量化环境干预措施或突发事件的健康影响)及时空分层病例交叉设计(兼顾研究结局在时间和空间上的关联性)。该项目的研究发现不仅为气候变化背景下的重点地区精神分裂症防控和预警提供新证据,而且在方法学上为其它精神疾病的人群研究提供了新思路和解决方案。目前,依托项目发表在Nature Mental Health、BMJ Global Health和中华疾病控制杂志等中文和英文论文共19篇,培养3名硕士研究生顺利毕业,研究结果多次在国内外学术会议上报告和展出,参编气候变化与人群健康相关书籍1次。
