内皮细胞是血脑屏障的关键组成部分,与周围细胞互作以维持血管稳态。内皮损伤是脑小血管病的起始事件,可引发血管内炎症和血脑屏障破坏,进而导致相关脑损害。本团队前期从中国国家卒中登记III研究中发现,PDE3A基因与脑小血管病相关;研究表明该基因参与调节内皮屏障功能与维持血管完整性。由此我们推测:PDE3A变化可引起内皮细胞激活和损伤,从而导致血脑屏障破坏及脑小血管病相关病理改变。而PDE3A参与内皮损伤及脑小血管病发生发展的机制有待深入研究。本项目拟采用CRISPR-Cas9技术分别构建PDE3A功能缺失和增强的模型鼠;在不同时间点(鼠龄第4-72周),评估内皮及脑小血管病相关表型及病理变化,评估血管炎症、血脑屏障完整性、胶质细胞变化及内皮细胞中基因表达差异,发掘相关分子网络,揭示PDE3A通过内皮细胞调控血脑屏障功能参与脑小血管病发生发展机制,为寻找脑小血管病新治疗靶点提供科学依据。

Endothelial cells are key components of the blood-brain barrier and interact with surrounding cells to maintain vascular homeostasis. Endothelial injury is the initial event of cerebral small vessel disease, which can lead to intravascular inflammation, destruction of the blood-brain barrier, and related brain damage. We have previously found that PDE3A gene was associated with cerebral small vessel disease from the China National Stroke Registry III (CNSR-III). PDE3A gene plays significant roles in regulation of endothelial function and maintenance of vascular integrity. Thus, we hypothesize that alteration of PDE3A can cause activation and injury of endothelial cells, which will lead to damage of blood-brain barrier and pathological changes related to cerebral small vessel disease. Nevertheless, the mechanism of PDE3A leading to endothelial dysfunction and cerebral small vessel disease remains to be further investigated. In this study, we intend to use CRISPR-Cas9 tool to generate mouse models of PDE3A function deletion or enhancement, respectively. Endothelial function, phenotypes and pathological changes related to cerebral small vessel disease will be assessed, at different time points (4-72 weeks of age). The differences of vascular inflammation, blood-brain barrier integrity, glial cells and gene expression in endothelial cells among different mouse models will also be evaluated. These researches will help to reveal the mechanism of PDE3A promoting the occurrence and development of cerebral small vessel disease through endothelial injury, and provide a substantial basis for searching for new therapeutic targets.
PDE3A参与调节内皮屏障功能、维持血管完整性、调节心肌收缩及血管扩张等功能,在心血管系统及神经系统等多领域发挥重要作用。临床研究发现,PDE3A基因突变与脑小血管病及内皮功能相关,但是其在脑小血管病发生发展中的作用机制尚不清楚。本研究发现多个PDE3A基因突变位点与脑小血管病及其影像特征相关。通过对PDE3A基因编辑小鼠的表型评价发现,与野生型小鼠相比,PDE3A-/-小鼠和PDE3A+/-小鼠在出生3个月时并没有表现脑小血管病相关表型。在慢性缺血BCAS造模后,PDE3A-/--BCAS小鼠有明显的认知功能损伤、自由探索能力下降、影像学特征分数各向异性值的下降、及髓鞘损伤;而PDE3A+/--BCAS小鼠中认知功能轻微损伤、自由探索能力轻微下降、影像学特征分数各向异性值轻微下降、及髓鞘轻微损伤,但与PDE3A+/--SHAM小鼠没有统计学差异。以上结果提示降低PDE3A表达可能会抑制慢性缺血小鼠的疾病负担,但是完全缺失PDE3A可能会加剧慢性脑缺血小鼠的疾病负担。采集PDE3A-/-鼠、PDE3A+/-鼠及WT鼠SHAM组和BCAS组的脑组织胼胝体区的样本进行bulk转录组测序。数据分析表明,PDE3A缺失的小鼠,在慢性缺血后,可能通过引起脑内血管生成及稳定相关分子的表达降低、炎症反应(如胶质细胞激活、补体系统级联反应及中性粒细胞相关)相关因子升高,从而进一步损伤血管结构和白质纤维结构,导致脑小血管病相关表型的加重。进一步的炎症标志物与脑小血管病相关性研究表明炎症标志物(尤其是中性粒细胞)水平的升高与脑小血管病的发生、严重程度及影像特征密切相关;遗传学角度也揭示了中性粒细胞与脑小血管病结局具有因果关联。本研究的发现为今后挖掘脑小血管病防治的潜在靶点提供依据。
