肺癌是我国发病率和死亡率最高的恶性肿瘤。由于缺乏灵敏的早期诊断手段,导致患者就诊晚,总体预后不佳。本课题组自主研发了超灵敏肺部MRI系统,在国内首次实现了信号增强50000倍以上的人体肺部MRI,为探测早期肺癌的通气结构和功能提供了新的技术平台。基于以上研究成果,本项目拟制备PFOB@PLGA这一生物相容性高的多模态分子探针,并利用红细胞作为载体,以降低探针的免疫原性和肝肾蓄积,将其高效运输至肺腺癌模型鼠的肺部,通过129Xe/19F双核MRI及荧光成像,追踪肺癌演进相关的肺通气和气血交换功能异常信息,为肺癌的早诊提供灵敏、多视角、无辐射、可持续监测的新型影像学手段。对多分子影像精准定位出的病灶,应用生物信息学及分子生物学方法,进一步探究与肺癌演进相关的异常信号通路与免疫微环境改变,为肺癌的早诊提供理论依据。
英文摘要
Lung cancer is the most common cancer and the leading cause of cancer-related mortality in China. Due to the lack of sensitive early diagnosis methods, 70-80% of lung cancer patients are diagnosed in the late stages, resulting in a poor overall survival rate. We have applied the ultra-sensitive 129Xe MRI to the human lung for the first time in China, providing a new technology platform for detecting the microstructure and function of early lung cancer. Based on the platform, we intend to develop a fluorescent-labeled PFOB@PLGA nanoformulation as the multi-modal molecular probe for 129Xe/19F dual-modal MRI and fluorescence imaging, which has high biological compatibility. To reduce the immunogenicity of the nanoparticles and their accumulation in the liver and kidney, we will use red blood cells as carriers to deliver high concentrations of PFOB@PLGA nanoparticles to the lung of lung adenocarcinoma model mice, namely "RBC-hitchhiking (RH)". Then we could perform 129Xe/19F dual-modal MRI and fluorescence imaging of lung cancer and quantitatively evaluate pulmonary gas-exchange function and lung microstructure of mice with lung cancer. The RH-PFOB@PLGA mediated 129Xe/19F MRI of lung cancer could provide a comprehensive, sensitive and non-ionizing radiation imaging method for early diagnosis of lung cancer. Moreover, lesions accurately located by multi-molecular imaging will be resected. We will use the lesions to explore the abnormal signal pathways and immune microenvironment changes related to lung cancer progression by bioinformatics and molecular biology methods, which could provide a theoretical basis for the early diagnosis of lung cancer.
阐明肺癌演进的分子机制对于肺癌的诊断和治疗具有重大意义。项目负责人所在团队研发的肺部超灵敏磁共振成像(129Xe MRI)系统于2023年全球首次获批进入临床,这一创新仪器能够对肺部疾病进行无创精准检测,“点亮”传统MRI的肺部盲区,从而无辐射、定量、可视化肺部的通气和交换功能。本项目应用这一系统进行了影像医学、生物信息学、分子生物学等多学科交叉研究,追踪了肺癌患者及模型小鼠演进中气-血交换功能的变化,并探究了癌症十大Hallmark特征之一——炎症对于气-血交换功能的调控作用。我们发现:一、非小细胞肺癌(NSCLC)患者的气-血交换功能在疾病早中期而非晚期即已受损,并在两种原位NSCLC小鼠模型中得到了进一步验证;二、通过单细胞RNA测序(scRNA-seq)和免疫组织化学(IHC)分析,发现NSCLC演进过程中气-血交换功能的损伤与早中期的肺癌炎症微环境相关,其特征为炎症因子IL-6的高表达以及肺泡间隔中巨噬细胞的显著富集。并且,气管滴注IL-6可显著增加健康小鼠肺泡间隔中巨噬细胞的富集,并损害其气-血交换功能;三、进一步地,在NSCLC早中期靶向IL-6/JAK信号通路,可显著增强MEK抑制剂的抗肿瘤效果,并减轻炎症相关的肺交换功能损伤。综上所述,本项目应用129Xe MRI探究了肺癌演进的分子机制,发现NSCLC演进早中期炎症导致的气-血交换功能损伤,能够作为肺癌早中期的炎症微环境形成的标志物,指导新型炎症靶向治疗方案的开发及时间窗选取,为129Xe MRI系统的进一步临床应用提供了理论基础、为肺癌演进分子机制相关研究提供了新视角和新技术。此外,还开发了1种1H/19F双模态MRI分子探针,可特异性响应半胱氨酸蛋白酶Caspase-3,可用于活体肿瘤的疗效评估。以上研究成果形成论文1篇(Analytical chemistry, under revision);1篇论文准备中;并已申请发明专利2项。
