疼痛不仅严重影响了人们的生活质量,而且造成了巨大的社会和经济负担。非侵入性神经调控技术作为一种安全、易操作的调控手段可以避免药物镇痛所伴随的副作用,有效缓解患者疼痛。目前,经皮神经电刺激和经颅直流电刺激均已作为一种有效的调控手段应用于镇痛领域,但两者的最优刺激参数和镇痛机制尚不清晰;而经颅交流电刺激作为一种潜在的镇痛方法其镇痛效果有待进一步验证。基于此,本项目拟联合采用量表、行为任务、脑电和磁共振成像技术等多种测量手段,考察非侵入性神经调控技术的镇痛可靠性和可推广性。具体工作包括:1、评估经皮神经电刺激、经颅直流电刺激和经颅交流电刺激的镇痛效果,探索三种镇痛技术的最优刺激参数;2、研究三种技术镇痛的神经机制;3、考察组合刺激效果,开发整合性调控方案;4、开发个性化的治疗方案,检验调控方案用于缓解慢性疼痛的有效性和持续性。本项目的开展将促进神经调控技术基础研究的临床应用,有效缓解患者的痛苦。

Pain is an unpleasant sensory and emotional experience associated with actual or potential tissue damage, which negatively impacts our lives, leading to an emotional and financial burden for patients, families, and the whole society. Due to many side effects of pain killers, it is necessary to find safe, effective, non-addictive analgesic strategies for pain management. Non-invasive neuromodulation techniques show tremendous promise in relieving pain due to its non-pharmacological and easy-to-use nature. Accumulating empirical evidence suggests that transcutaneous electrical nerve stimulation (TENS) and transcranial direct current stimulation (tDCS) are effective in reducing pain. However, the knowledge about their optimal stimulation parameters and underlying mechanisms is still limited. In addition, the analgesic effect induced by transcranial alternating current stimulation (tACS) has not been fully testified. To achieve a better understanding of the analgesic effect and neural mechanisms associated with non-invasive neuromodulation techniques, the present project examines the analgesic effect of the three neuromodulation techniques using a variety of measurements, including questionnaires, behavioral assessments, electroencephalography (EEG), and functional magnetic resonance imaging (fMRI). Specifically, we first evaluate the analgesic effect of TENS, tDCS, and tACS by investigating their optimal parameters to maximize the analgesic effect. Second, we explore the underlying neural mechanisms of TENS-, tDCS-, and tACS-induced analgesia. Third, we optimize the analgesic effect by combining the three neuromodulation techniques. Finally, we develop a personalized treatment program based on findings obtained in previous steps and further validate its analgesic effect in patients with chronic pain. In summary, this project will not only deepen our understandings of the analgesic effect associated with non-invasive neuromodulation techniques but also boost the translation of basic studies to clinical applications.
非侵入性神经调控技术因其安全性和易操作调控性,在疼痛管理领域展现出重要应用潜力,但不同技术的最优刺激参数和镇痛机制仍需深入探究。为解决这些关键问题,本项目采用心理量表、行为任务、脑电和磁共振成像等技术手段,评估了经皮神经电刺激(TENS)、经颅直流电刺激(tDCS)和经颅交流电刺激(tACS)在不同刺激参数下的镇痛效果差异;探究了TENS、虚拟现实和运动的镇痛效果及机制;开发了TENS与虚拟现实同步联合的镇痛方案,并考察了其镇痛效果和机制;总结了神经调控技术的重要研究进展,展望了跨学科交叉融合背景下神经调控技术在应用和转化中存在的机遇与挑战。此外,为探究神经调控镇痛的刺激靶点,本项目深入挖掘了疼痛特异的神经标志物,并考察了临床急慢性疼痛的病理机制和预测因素。结果显示:(1)tACS调控前额叶/初级体感/顶枕皮层的alpha或gamma神经振荡对辣椒素诱发长时疼痛无显著镇痛效果, 调控顶枕皮层的gamma振荡对急性疼痛具有即时镇痛效果;(2)tDCS调控前额叶/初级体感皮层对急性疼痛和辣椒素诱发的长时疼痛均无镇痛效果;(3)TENS和虚拟现实分别通过脊髓抑制、皮层-脑干-脊髓网络和调控前扣带皮层和岛叶等疼痛相关脑区的神经振荡来发挥镇痛作用,虚拟现实与TENS同步联合可在最大程度上缓解被试的疼痛感觉和不愉悦度,降低负性情绪;(4)运动强度与镇痛效果密切相关,且运动镇痛效应通过自传入原理、非伤害性干扰及疼痛下行抑制调控机制共同作用;(5)初级体感皮层中间神经元驱动的疼痛诱发gamma振荡信号是痛觉强度的关键编码机制,丘脑背内核及其与前扣带皮层和脑岛通路可特异性表征疼痛信息,疼痛诱发大脑反应可作为疼痛分辨力稳定且特异的神经指标;(6)术前大脑自发神经振荡与术后痛强度密切相关,偏头痛患者存在注意功能损伤,其脑功能存在异质性且与针刺疗效个体差异有关。本项目的研究成果不仅有助于促进神经调控基础研究的临床应用,也加深了对于疼痛产生机制的理解。
