早发性卵巢功能不全(Premature ovarian insufficiency,POI)是指女性40岁之前出现卵巢功能减退,发病率约为1%-5%,是女性不孕症的主要病因之一。随着社会发展和女性生育年龄推迟,POI发病率呈逐年上升趋势,不仅影响女性身心健康,也成为影响社会人口增速和结构的重要因素。为拓展对POI遗传学病因的认识,我们对50例原发性POI患者进行了全外显子组测序,并首次发现同源重组基因EXO1和BRCA2的高致病风险突变。前期研究发现EXO1突变能够影响酵母减数分裂和体细胞的同源重组修复;最新构建的BRCA2点突变小鼠在出生后卵母细胞迅速丢失,该模型与既往报道的差异提示基因组水平的BRCA2缺陷将导致更早阶段的生殖细胞发育异常。本项目拟借助携带人源突变的小鼠模型,进一步研究EXO1和BRCA2突变在卵母细胞减数分裂和卵泡发育中的作用,探讨其导致POI发生的具体机制。

Premature ovarian insufficiency (POI) is characterized by cessation of menstruation occurs before the age of 40 years. Approximately 1%-5% of women are affected by POI, which is one of the causes of female infertility. With the social changes and postponed childbearing plan of women, the incidence of POI is increasing year by year, which not only affects the physical and mental health of women, but also becomes an important factor affecting the growth rate and structure of the population. To expand the understanding of the genetic etiology of POI, we performed whole exome sequencing in 50 patients with primary POI, and identified mutations in homologous gene EXO1 and BRCA2. Our studies showed that the EXO1 mutation disturbed meiosis of yeast and homologous recombination efficiency of human cell line. The point mutant mouse model demonstrated progressive loss of oocytes, which was different from previous report, indicating genomic BRCA2 defects might affect germ cell development at earlier stage. In the project, we attempt to illustrate the influence of EXO1 and BRCA2 on oogenesis and follicle development using the point mutant mouse, furthermore, explore the role of homologous recombination pathway played in pathogenesis of POI.
早发性卵巢功能不全(Premature ovarian insufficiency,POI)是指女性40岁之前出现卵巢功能衰竭,是最常见的女性生殖衰老性疾病。由于POI发病隐匿、进展迅速且无法逆转,多数患者就诊时已丧失自卵生育机会。研究POI的发病机制是推动POI早诊早治的关键。遗传因素是POI的重要致病因素之一,我们前期利用全外显子组测序在50例原发性闭经POI患者中发现BRCA2和EXO1基因突变。随后,依托本项目,进一步探究了以上基因突变导致POI的病理机制。首先,通过对携带人源Brca2复合杂合突变的小鼠模型进行生殖表型观察、卵泡发育和减数分裂检测,发现Brca2复合杂合突变通过抑制减数分裂同源重组,导致出生后卵母细胞迅速凋亡,引发青春期前POI。通过观察小鼠的肿瘤发生情况及检测体细胞DNA损伤修复功能,发现Brca2复合杂合突变在卵巢功能丧失后仍增加肿瘤发生风险,且与体细胞的DNA损伤修复能力下降有关。同时,利用Exo1同源突变酵母实验,证实EXO1突变对减数分裂的影响。随后,进一步扩大POI队列,利用全外显子组测序绘制散发性POI的变异全景图,将POI的基因突变发现率提升至23.5%,发现在散发性POI患者中,BRCA2和EXO1的突变携带率分别为0.5%和0.29%。此外,进一步围绕减数分裂相关基因变异在POI中的致病作用及机制拓展研究,先后发现MEIOSIN、PRDM9和KASH5基因突变分别通过影响卵母细胞减数分裂启动、双链DNA断裂、同源染色体配对等过程,导致减数分裂阻滞,引发卵母细胞凋亡和POI。以上研究结果进一步丰富和完善了减数分裂相关基因缺陷在POI中的作用,为POI病因诊断及干预提供参考。
