毛细胞损失是造成前庭功能障碍的主要原因。前庭椭圆囊毛细胞在损伤后仅具有有限的自发性再生能力,且不能再生成熟毛细胞。申请人前期研究表明,在椭圆囊毛细胞损伤后,上调Wnt信号通路可以促进内耳支持细胞增殖,过表达Atoh1可促进支持细胞分化新生毛细胞。但单独调控Wnt或Atoh1却不能促进功能性毛细胞再生和前庭功能修复。本项目在前期研究基础上,拟通过AAV介导Wnt,Atoh1,Gfi1和Pou4f3协同调控内耳干细胞促进功能性毛细胞再生及前庭功能修复。拟实现目标:1.使用Pou4f3-DTR转基因小鼠建立小鼠毛细胞和前庭功能损伤模型。2.建立AAV介导的前庭椭圆囊基因治疗系统。3.以AAV为载体,协同调控Wnt,Atoh1,Gfi1和Pou4f3这4个信号通路。4.研究功能性毛细胞再生和前庭功能修复。本项目将为临床上以AAV为载体,通过基因治疗恢复前庭功能奠定理论和临床应用基础。

Dysfunction or loss of hair cells is the primary causes of balance dysfunction. Limited hair cell regeneration takes place in the mammalian utricles, while few acquires mature hair cell features. Preliminary studies have been showed that upregulation of Wnt signal pathway can induce supporting cell proliferation and overexpression of Atoh1 can promote supporting cells transdifferentiate to hair cells after hair cell damage in mouse utricle. However, no functional hair cell regeneration and vestibular function recovery have been observed by activating Wnt or Atoh1 alone. Based on our previous findings, this proposal will investigate functional hair cell regeneration and vestibular function recovery after AAV-mediated co-regulation of Wnt,Atoh1,Gfi1 and Pou4f3 signal pathways in damaged neonatal and adult mouse vestibular system. First, transgenic mice will be used to establish neonatal and adult mice utricle hair cell and vestibular function damage model. Then, different subtypes of AAV will be screened to build an AAV-mediated high transduction gene delivery system in mouse vestibular system. Next, AAV-β-catenin-GFP and AAV-Atoh1-Gfi1-Pou4f3 will be delivered to inner ear vestibular system. Lastly, functional hair cell regeneration and vestibular function recovery will be examined. This proposal will establish an AAV-mediated gene therapy system to induce hair cell regeneration and function recovery, focusing on translational research from basic science to clinical application with the goal of curing balance disorders resulting from inner ear vestibular hair cell loss.
前庭功能紊乱是一种严重影响人类健康的疾病,主要表现为眩晕、平衡失调、头晕,常伴随耳聋和耳鸣。内耳前庭系统对维持人体运动和平衡至关重要,其功能依赖于内耳前庭毛细胞,这些机械感受细胞是实现听觉和平衡的关键。然而,环境因素、药物副作用、疾病及年龄引起的毛细胞损伤或丢失是导致前庭功能障碍的主要原因。研究表明,非哺乳脊椎动物的前庭毛细胞可以通过持续增殖、转分化或直接分化等机制实现自发再生,而哺乳动物前庭毛细胞在损伤后仅表现出有限的自发再生能力。因此,促进前庭毛细胞损伤后的功能性再生已成为近年来内耳前庭研究的重点和难点。.申请人前期研究发现,在椭圆囊毛细胞损伤后支持细胞作为内耳干细胞能够有限再生毛细胞。既往研究显示,外源性ATOH1的过表达可促进毛细胞再生,但这些再生细胞难以完全成熟。近年来,腺相关病毒(AAV)凭借广泛的细胞向性、稳定的基因表达及极低或无耳毒性,逐渐被应用于内耳基因治疗领域。AAV介导的基因治疗为有效调控内耳干细胞,促进功能性毛细胞再生提供了有效的途径。.本研究首先对14个不同发育时间点的小鼠椭圆囊毛细胞进行了转录组分析,定义了发育及成熟阶段前庭毛细胞的特征。结合毛细胞发育过程中内源性ATOH1逐步下调的自然规律,设计了携带支持细胞特异启动子GFAP和RLBP1的AAV载体,以实现ATOH1过表达后的动态调控。在离体受损的椭圆囊中,CMV-ATOH1和GFAP-ATOH1相比RLBP1-ATOH1更显著地促进了毛细胞再生;同时,GFAP-ATOH1和RLBP1-ATOH1诱导的毛细胞表现出更接近成熟毛细胞的转录组特征。在体内受损的椭圆囊中,GFAP-ATOH1诱导再生的毛细胞表达II型成熟毛细胞的基因特征,并生成更多具有纤毛和突触的毛细胞。.本研究通过AAV介导的基因治疗,在毛细胞损伤后实现对ATOH1过表达的时空调控,验证了其在促进内耳成熟毛细胞再生中的关键作用。本项目采用AAV载体介导基因治疗与干细胞再生技术相结合的策略,为前庭功能障碍的基础研究提供了新思路,同时也为临床治疗提出了可行的解决方案,具有重要的理论价值和临床意义。
