项目申请人的前期研究结果显示,对于复发难治的急性B淋巴细胞白血病(B-ALL),靶向CD19的CAR-T细胞在患者体内的长期存续可显著减少治疗后的复发,探索维持CAR-T细胞干性特征的技术手段和机制对于提高CAR-T的疗效具有重要意义。转录因子c-Myb对于细胞的分化和增殖具有重要作用,我们在预实验中观察到可在患者体内长期存续的CAR-T细胞高表达c-Myb,并伴随能量代谢和线粒体相关基因的上调,提示c-Myb可能通过影响细胞能量代谢维持CAR-T细胞的干性特征。本项目计划通过干扰或过表达CAR-T细胞的c-Myb,检测细胞表型及开展体外杀伤实验和长期扩增培养实验,明确c-Myb对于CAR-T细胞的影响,并进一步在动物实验中验证其作用,最后从细胞能量代谢角度探索c-Myb维持CAR-T细胞干性的具体机制。本项目将为提高CAR-T的体内存续提供一种新方法,并为c-Myb的研究提供新的视角。

Results from applicant’s previous studies show that, for relapsed/refractory acute B lymphoblastic leukemia patients, long-term persistence of CD19-specific CAR-T cells significantly reduces the post-treatment relapse. Exploring the mechanism and strategies to maintain CAR-T cell stemness is significantly important to enhance the clinical outcomes of CAR-T therapy. Transcription factor c-Myb plays critical roles during cellular differentiation and proliferation. In our preliminary experiments, we observed elevated expression of c-Myb in CAR-T samples with longer in vivo persistence, concomitant with up-regulation of genes involved in energy metabolism and mitochondria regulation. This finding suggests that c-Myb may help maintain CAR-T stemness through the regulation of cellular energy metabolism. . In this proposal, we first plan to confirm our previous findings: how modulation of c-Myb expression levels, such as by RNA interference or overexpression, affects CAR-T cell phenotypes and in vitro target killing. Long-term in vitro expansions, the anti-tumor performance of CAR-T cells with different c-myb expression levels in tumor-bearing animal models, and the potential mechanism will be further studied. We hope to clarify the specific mechanism of how c-Myb maintains CAR-T stemness through energy metabolism. This research may provide a new approach to enhance the in vivo persistence of CAR-T cells. It also reveals a new strategy of c-Myb study.
靶向CD19的嵌合抗原受体T淋巴细胞(chimeric antigen receptor T,CAR-T)在复发难治的急性B淋巴细胞白血病的治疗中展现了突破性的进展,但治疗后的复发是严重影响患者长期生存的一个主要因素,积极探索新的技术手段减少CAR-T治疗后的复发具有极为重要的临床意义和社会经济价值,而延长CAR-T回输后的体内存续可能可以减少白血病治疗后的复发。项目申请人前期通过设计基因panel,对患者回输剩余并冻存的CAR-T细胞进行基因表达检测,以体内存续3个月时间为界,发现可在患者体内长期存续和不可长期存续的CAR-T细胞在c-Myb的表达上有较显著的差异,进一步的检测发现c-Myb的表达上调伴随能量代谢相关基因的上调,提示c-Myb可能是提升CAR-T存续的一个关键因子。.通过本项目的实施,课题组通过分析临床数据,确认了CAR-T的长期体内存续是确保白血病的CAR-T治疗后长期缓解的重要因素,并通过临床标本的检测确认了c-Myb在长存续CAR-T细胞中的表达优势。通过开展体外实验对敲降和过表达c-Myb的CAR-T细胞进行细胞表型和功能比较的研究显示c-Myb可能通过抗凋亡提升CAR-T细胞体内扩增和存续。并基于研究发现,通过优化细胞培养方式构建了Prime CAR-T细胞制备体系,在动物实验中显示了其相对于普通CAR-T更好的肿瘤控制,提示良好的临床应用前景。.截止课题结题,共发表SCI论文2篇(中科院1区论文1篇,中科院2区论文1篇),培养硕士研究生2人,举办省级会议2场,参加国际会议3场。
