死亡受体5(DR5),也称肿瘤坏死因子相关凋亡诱导配体受体2(TRAIL-R2),因其选择性地肿瘤细胞杀伤作用成为肿瘤治疗的热门靶点。多项临床试验显示其靶向治疗在部分患者中表现出显著的抗肿瘤效果,但II期临床试验却均未取得预期成功。表明其在未筛选患者中的临床试验难以成功,亟需探索一种筛选获益患者的有效方法。本项目使用新型长半衰期核素Zr89标记其特异性抗体CTB006构建靶向分子探针89Zr-CTB006,在细胞水平对其对DR5的靶向特异性、作用关系及表达水平的定量进行研究。并通过PET/CT的高分辨及定量等能力,对其定量检测DR5表达的能力进行验证。总之,本项目构建的分子靶向探针,从分子水平,可验证DR5单抗与其结合的特异性,并获得其激动细胞凋亡的相关作用机制;从肿瘤临床免疫显像方面,可提供肿瘤DR5表达水平的无创性定量检测方法,指导DR5靶向肿瘤治疗中获益患者筛选。

Death receptor 5 (DR5), also known as tumor necrosis factor related apoptosis inducing ligand receptor 2 (TRAIL-R2), has become a hot target of tumor therapy because of its selective killing effect on tumor cells. A number of clinical trials showed that DR5 targeting therapy showed significant antitumor effect in some patients, but no phase II clinical trials succeed. Therefore, it is difficult to succeed in clinical trials without screening patients and it is urgent to explore an effective method for screening patients who would benefit from DR5 targeting therapy. In this research, we would use a new long half-life nuclide 89Zr to label DR5 specific antibody CTB006 to construct a DR5 targeting molecular probe 89Zr-CTB006. And its targeting specificity, functional relationship and DR5 expression levels would be studied at the cell level. The ability of PET / CT to detect DR5 expression would be verified with its high resolution and quantitative ability. Take together, the molecular targeted probe constructed in this project would verify the specificity of DR5 monoclonal antibody and its combination at the molecular level, and obtain the relevant mechanism of its activation of apoptosis. From the aspect of tumor clinical immunoimaging, it would provide a non-invasive quantitative detection method of DR5 expression levels in tumor lesions and screening of patients who would benefit from DR5 targeted antitumor treatment.
本研究开发了89Zr标记的DR5靶向探针(89Zr-CTB006),并探索其在肿瘤免疫治疗中的应用。DR5(死亡受体5)在多种肿瘤中高表达,通过外源性凋亡途径显著抑制肿瘤进展,因此成为免疫治疗的重要靶点。然而,现有DR5抗体疗法在未筛选患者中疗效有限,缺乏有效的疗效预测手段,制约了其临床转化。本研究通过利用PET分子影像技术,旨在实现患者的精准筛选、疗效预测和疗效监测,从而提高DR5抗体治疗的效果。研究团队成功制备了89Zr-CTB006,并优化了放射化学标记工艺,确保其放射化学纯度高于95%,同时具备良好的稳定性和生物活性。临床前实验中,89Zr-CTB006显示出对DR5的高亲和力和靶向结合能力,在DR5阳性肿瘤细胞中显著富集,验证了其在细胞水平的靶向特性。荷瘤小鼠模型中的实验结果表明,探针在DR5高表达肿瘤中的显像能力强,且探针的动态分布与受体表达水平高度一致,进一步证明了其在体内的准确性和可靠性。更为重要的是,本研究首次在人体中进行了89Zr-CTB006探针的临床验证。通过在21例胃肠道肿瘤患者中进行PET/CT显像,研究发现超过50%的患者肿瘤DR5表达低于预期,显示了该探针在精准筛选患者中的重要作用。PET/CT显像能够精准地区分DR5高低表达患者,SUVmax值与RNAscope检测结果显著相关,灵敏度为100%,特异度为92.9%。此外,研究还发现转移病灶也表现出显著的89Zr-CTB006摄取,肿瘤摄取水平与DR5表达水平呈正相关,已接受放化疗的患者肿瘤摄取显著低于未治疗患者,提示该探针在疗效监测中的潜力。总体而言,本研究首次实现了DR5靶向PET探针在人体中的临床应用验证,为靶向抗体治疗的患者筛选、疗效预测及治疗方案调整提供了科学依据。89Zr-CTB006探针能够无创、动态、定量地评估患者肿瘤DR5表达水平,从而提高靶向抗体疗法的疗效,避免不必要的治疗风险,并推动DR5抗体疗法的临床转化。此外,89Zr-CTB006探针为肿瘤免疫治疗的个性化诊疗提供了新工具,具有显著的临床转化潜力,为肿瘤免疫治疗的精准诊疗开辟了新路径。
