脑外伤(TBI)不仅造成神经功能障碍,还破坏骨骼代谢系统,增加病人骨折的风险。临床研究证实:相比单纯骨折,脑外伤伴骨折的复合损伤(MI)可以加速骨折愈合。但是,相比TBI,MI对神经功能的影响仍然存在争议。预实验发现,相比TBI,MI可以抑制脑组织炎症反应和神经元死亡。因此,本项目将运用多种生物学方法,以TRAP阳性的免疫细胞为切入点,拟解析:(1)TRAP在MI中脑损伤病理过程中的角色;(2)TRAP调控MI中脑组织程序性细胞死亡的机制;(3)TRAP对TBI致神经功能障碍的影响及机制。最终阐明通过精准激活TRAP,激发免疫细胞更强的吞噬功能,避免过度炎症(神经炎症)和神经细胞死亡,促进神经功能的恢复。以上研究成果拟证实MI中脑损伤更轻微的结果及潜在机制,提出精准激活免疫细胞治疗TBI的新理念和有效靶点(TRAP)。

Traumatic brain injury (TBI) leads to the dysfunction of central nervous system, as well as the destruction of skeletal metabolic system, thus increasing the risk of bone fracture. Previous clinical studies have found that TBI with fracture (MI) could accelerate fracture healing compared with isolated fracture. However, comparing with isolated TBI, the effect of MI on neurological function is still remained controversial. Our pilot study found that MI inhibited the neuroinflammation and neuronal death in the acute phage compared with TBI only. Therefore, this project will use a variety of biological methods, and focus on the tartrate resistant acid phosphatase (TRAP) positive immune cells to explore and analyze: (1) the role of TRAP in the pathological process of brain injury in the MI model; (2) the regulatory mechanism of TRAP on programmed cell death in the brain tissues of the MI model; (3) the mechanisms of TRAP in regulating neurological deficits post-TBI. This project will finally clarify that the phagocytosis mediated by TRAP activated precisely is enhanced to avoid excessive inflammation (neuroinflammation) and brain damage (pyroptosis), and consequently the promoted recovery of neurological functions. These results will confirm the fact that brain injury is slighter in the MI model and its potential mechanisms, and provide a new concept which is activating immune cells precisely and a novel target (TRAP) for clinical TBI treatment.
临床流行病学研究已经证实脑外伤能够加速骨折愈合,但是骨折对脑外伤的影响仍不明确。在本项目的资助下,我们分别建立创伤性颅脑损伤、胫骨骨折、创伤性颅脑损伤合并胫骨骨折的小鼠模型,重点研究骨折对脑外伤的影响。影像学结果验证了“脑外伤加速骨折愈合”的事实;同时,我们发现伴随的骨折能够减轻脑外伤后急性期神经细胞变性,缓解脑水肿,抑制焦亡相关蛋白的表达,减少神经元性炎症小体的数量,增强了恢复期脑源性神经营养因子的表达,从而促进了脑外伤后的神经功能恢复。为了探究骨折促进脑外伤后神经功能恢复的作用机制,依据转录组测序结果,我们首先将研究目标锁定在破骨细胞:在颅脑损伤合并骨折的动物模型中,通过药物干预阻断破骨细胞活化后,我们发现抑制破骨细胞激活不仅会延缓骨折愈合,还会促进脑组织中焦亡相关蛋白的表达,增加神经元性炎症小体的数量,进而延缓神经功能恢复;而在颅脑损伤模型中,使用转基因小鼠靶向激活破骨细胞则能够降低细胞死亡,抑制焦亡相关蛋白的表达,减少神经元性炎症小体的数量,从而促进脑外伤后神经功能恢复。以上结果表明破骨细胞也是影响脑外伤发病机制的重要细胞类型之一。随后,依据蛋白组测序和空间转录组测序的结果,以破骨细胞为切入点,我们进一步筛选到与其活化息息相关的抗酒石酸酸性磷酸酶,并研究了该酶在不同损伤模型中的表达规律,结果表明抗酒石酸酸性磷酸酶与脑外伤的病理进展密切相关;伴随的骨折通过对该酶的调控可能是介导骨折促进脑外伤后神经功能恢复的机制之一。最后,我们构建了抗酒石酸酸性磷酸酶腺相关病毒,通过一系列行为学实验证明了抗酒石酸酸性磷酸酶可以作为脑损伤治疗的新型靶点。依据不同损伤动物模型之间的恢复差异,本项目首次明确了骨折对脑外伤恢复的正向作用,揭示了破骨细胞及其分泌的抗酒石酸酸性磷酸酶对脑外伤预后的重大影响以及将该酶作为脑外伤治疗靶点的可行性。此外,也可能为骨质疏松病人遭受脑外伤的治疗提供理论指导。
