创面炎症微环境是造成慢性创面难愈合的重要原因。表皮角质形成细胞在炎症、感染时开启抗原提呈细胞功能,激活T细胞,引发适应性免疫反应,参与炎症微环境的形成。但角质形成细胞抗原提呈功能在慢性创面愈合中的作用及调控其抗原提呈功能的分子机制尚不清楚。EGFR和MAPK抑制剂在抗癌时引起皮疹,提示EGFR/MAPK信号通路参与皮肤炎症反应。我们前期研究发现阻断该信号通路能在体外和体内促进角质形成细胞表达抗原提呈相关基因、摄取荧光抗原和抑制创面修复。本项目拟利用细胞模型、创面模型、EGFR敲除小鼠、通路阻断剂、高通量测序等研究EGFR/MAPK信号通路对角质形成细胞的抗原提呈相关基因的表达、抗原提呈过程、激活T细胞、创面炎症反应和创面愈合质量的作用及机制。该研究将破译角质形成细胞对创面炎症反应的作用及其抗原提呈功能的分子机制,为改善慢性创面愈合质量及防治皮肤免疫炎症性疾病提供新途径和思路。

Wound inflammatory microenvironments contribute to the chronic wound healing. Under the inflammatory or infectious conditions, epidermal keratinocytes can function as antigen-presenting cells to activate T cells and initiate adaptive immune responses, constituting the inflammatory microenvironments. However, the roles of antigen-presenting functions of keratinocytes in the chronic wound healing and the molecular mechanisms of regulating the antigen-presenting functions of keratinocytes remain unclear. EGFR and MAPK inhibitors can cause skin rashes when used to treat cancers, indicating that the EGFR/MAPK signaling pathway may be associated with skin inflammatory responses. Our previous studies show that inhibiting this signaling pathway can increase the expression of antigen presentation-associated genes, fluorescent antigen uptake in keratinocytes in vitro and in vivo and impair the wound healing. By using cellular models, wound models, EGFR knockout mice, signaling pathway inhibitors, and high-throughput sequencing, the present study aims to investigate the roles and mechanisms of the EGFR/MAPK signaling pathway in regulating the expression of the antigen-presentation-associated genes in keratinocytes, antigen presentation process, and activation of T cells, wound inflammation, and wound healing. This study will discover the contribution of keratinocytes to wound inflammation and the molecular mechanisms underlying their antigen-presenting functions, and will offer new strategies and ideas for improving chronic wound healing as well as preventing and curing skin inflammatory diseases.
慢性创面过度炎症是糖尿病创面难愈的核心机制。表皮角质形成细胞(KCs)是再上皮化过程中的关键执行者,表皮角质形成细胞(KCs)作为再上皮化的核心细胞,其免疫调控功能尚未完全阐明。临床发现EGFR/MAPK抑制剂常引发皮肤炎症,提示该通路可能在KCs的免疫调控中发挥重要作用。本项目旨在解析EGFR/MAPK信号通过调控KCs促炎表型加重创面炎症的分子机制,为糖尿病创面治疗提供新策略。建立正常C57BL/6J小鼠和db/db糖尿病鼠全层皮肤缺损模型,发现EGFR抑制剂厄洛替尼(100mg/kg/d)处理显著延缓两组创面愈合,并导致新生表皮异常增厚及终末分化障碍。免疫细胞浸润分析显示厄洛替尼显著增加创面巨噬细胞和T细胞等炎症细胞浸润。RNA-seq分析发现,厄洛替尼处理显著上调与白细胞趋化、粒细胞迁移相关的促炎基因,且增强IL-17、细胞因子-细胞因子受体互作和NF-κB等炎症信号通路,同时下调表皮分化相关基因的表达。关键枢纽基因分析揭示,厄洛替尼显著上调的S100A8/A9在蛋白互作网络中处于核心位置,且在糖尿病创面中的表达较正常创面高。体外实验中,厄洛替尼显著上调人表皮角质形成细胞(NHEK)中S100A8/A9的表达,同时伴随CCL17、CXCL2/3/20、CCL4、NFKB、CIITA等促炎因子的显著升高。S100A8/A9抑制剂Paquinimod可有效逆转上述促炎因子表达,表明EGFR/MAPK通过S100A8/A9调控炎症级联反应。本研究揭示表皮角质形成细胞通过EGFR/MAPK-S100A8/A9轴调控创面免疫微环境的新机制,为理解表皮细胞免疫调控功能提供了理论基础。由于S100A8/A9在糖尿病创面中高表达,该研究为开发糖尿病慢性创面的双靶向治疗策略提供了重要的理论依据,也为逆转EGFR抑制剂引发的皮肤毒性提供新靶点。意外发现长期高剂量厄洛替尼可诱导小鼠腹膜间皮瘤,为EGFR-TKI耐药机制及间皮瘤发病研究提供了新模型。
