高糖诱导巨噬细胞持续M1极化介导的慢性炎症反应是影响II型糖尿病患者种植体骨愈合障碍的关键原因。前期研究发现将血小板衍生生长因子BB(PDGF-BB)加载至TiO2纳米管表面可诱导高糖条件下巨噬细胞M2极化、改善炎症反应,但具体机制不明。细胞膜表面受体是介导细胞感受外源刺激、调控胞内信号传导的重要媒介;PDGF-BB@TiO2可磷酸化激活M1型巨噬细胞膜表面的β型受体(PDGFR-β),并诱导胞内miR-143-3p表达上调,而阻断受体后巨噬细胞M2极化减弱。因此我们推测,PDGFR-β是调控糖尿病巨噬细胞M2极化的重要靶点。本项目拟利用细胞分生、动物模型等手段阐明PDGF-BB@TiO2通过PDGFR-β介导的miR-143-3p/PTEN途径调控糖尿病种植体周巨噬细胞M2极化的新机制,为研究膜受体介导的细胞功能调控提供理论依据,也为针对II型糖尿病患者的种植体表面优化设计提供新思路。

The chronic inflammatory response mediated by continuous M1 polarized macrophages which induced by high glucose stimulation is the key factor affecting the defect of implant bone healing in patients with type II diabetes mellitus. Previous studies have found that loading platelet-derived growth factor BB (PDGF-BB) on the surface of TiO2 nanotubes can induce M2 polarization of macrophages and improve the inflammatory response under high glucose environment, but the specific mechanism is unclear. Cytomembrane surface receptors are important mediators to mediate the exogenous stimuli and regulate the intracellular signal transduction. PDGF-BB@TiO2 nanotubes can phosphorylate and activate the β-receptor of PDGF (PDGFR-β) on the cytomembrane surface of M1 macrophages, thus inducing the up-regulation of intracellular miR-143-3p expression; while the M2 polarization of macrophages is weakened after blocking PDGFR-β. Therefore, we hypothesized that PDGFR-β is an important target of regulating M2 polarization in diabetic macrophages. This project intends to use cytobiological and molecular biological medthods, animal models and other means to clarify the new mechanism of PDGF-BB@TiO2 nanotubes regulating M2 polarization of diabetic peri-implant macrophages through the miR-143-3p/PTEN pathway mediated by PDGFR-β, so as to provide a theoretical basis for the study of cytomembrane receptor-mediated cell function regulation, and also provide a new idea for the optimization design of implant surface in patients with type II diabetes mellitus.
近年来,种植修复已成为缺牙患者的首选治疗方案,但糖尿病患者的种植牙却面临着骨整合不良等诸多风险。研究表明,持续M1极化的巨噬细胞介导的慢性代谢性炎症反应可能是导致机体与材料骨整合障碍的关键原因。因此,以改善糖尿病特殊微环境为目标的种植体材料功能化修饰逐渐成为前沿热点。.本研究中,我们利用阳极氧化+多巴胺中间涂层法构建了一种新型钛材料,即血小板衍生生长因子BB(PDGF-BB)功能化TiO2纳米管(PDGF-BB@TiO2),并利用场发射扫描电镜、原子力显微镜、接触角测量仪及X射线光电子能谱等对其表面形貌、粗糙度、亲水性和元素组成等理化性能进行评估;同时,蛋白释放试验明确了该材料对于PDGF-BB的缓释可达2周。通过细胞凋亡和增殖实验明确了该材料的细胞安全性。继而利用其培养巨噬细胞,发现可促进后者的早期黏附;进一步通过qRT-PCR、流式细胞术和细胞免疫荧光发现该材料可改善高糖对巨噬细胞M1极化的诱导作用,显著提升M2极化标志物的表达水平。.细胞膜表面受体在帮助细胞首先感知、接受外界材料刺激时发挥重要作用。本研究中,我们发现PDGF-BB@TiO2材料可显著提升β型PDGF-BB受体—PDGFR-β的磷酸化水平,利用AG1296抑制其表达和磷酸化后,发现PDGF-BB@TiO2材料对高糖条件下巨噬细胞M2极化的诱导作用也被抑制。同时,通过高通量测序和qRT-PCR筛选出miR-143-3p可能是PDGF-BB@TiO2材料诱导巨噬细胞M2极化的关键分子;进一步,利用AG1296和miR-143-3p mimics进行挽救实验,发现虽然AG1296可降低巨噬细胞M2极化,但miR-143-3p mimics的加入却可逆转上述作用。最后,我们利用高糖高脂饮食+链脲佐菌素注射法构建大鼠糖尿病模型,并将PDGF-BB@TiO2种植体植入大鼠股骨干骺端,发现该材料可显著提升骨整合效果,组织学分析发现种植体周围组织中浸润大量M2型巨噬细胞,同时高表达PDGFR-β。.总结,本项目成功构建针对糖尿病的PDGF-BB@TiO2种植体材料,并证实其对高糖环境下巨噬细胞M2极化的积极调控作用,同时阐明PDGFR-β/miR-143-3p信号轴参与上述调控的重要分子机制,为高糖环境下巨噬细胞M2极化的研究提供新靶点及新机制,更为针对糖尿病患者的种植体材料优化设计提供新理念和新思路。.
