阐明肺腺癌发生和转移的分子机理是当前肺腺癌研究的重点和难点。炎症与肺腺癌的发生和转移紧密相关,但具体机制尚不清楚。通过对我们前期进行的小鼠尾静脉接种肺腺癌细胞转移实验、细胞系mRNA表达谱分析及细胞培养上清液的细胞因子检测,我们提出“PTGES/AKT/G-CSF通路能够调控肺腺癌微环境促进肺腺癌的发生和转移”的科学假说。我们将进一步明确肺腺癌细胞炎症反应对肿瘤微环境中免疫细胞的调控作用,并拟在细胞、小鼠和肺腺癌临床样本三个层面上阐明:①PTGES对肺腺癌自发和转移的重要性;②PTGES调控肺腺癌免疫微环境的分子机制;③靶向PTGES联合PD-L1抗体治疗能够阻断免疫逃逸。本课题的实施将深化PTGES通过AKT/G-CSF这一信号通路调控肺腺癌发生和转移的认识,并能为抑制肺腺癌的发生和转移开拓新思路,明确其临床转化意义为肺腺癌分类、预后判断和治疗提供新的理论依据。
英文摘要
The most important and difficult point of lung adenocarcinoma (LADC) research is to clarify the mechanism of LADC spontaneous and metastasis. Inflammation is strongly related to LADC spontaneous and metastasis, but the molecular mechanism remains to be elucidated. Through tail vein injection of LADC cells in mice, cells mRNA seq analysis and cytokine detection of tumor cells conditioned medium, we hypothesize that “the PTGES/AKT/G-CSF pathway can regulate the tumor microenvironment to promote the occurrence and metastasis of LADC”. We will further clarify the regulation of LADC inflammation on immune cells in the tumor microenvironment, and plan to clarify at the three levels of cells, mice, and LADC clinical samples. In this proposal, we are going to study three specific aims: ①The importance of PTGES to LADC spontaneous and metastasis; ②The mechanism of PTGES regulating the immune microenvironment of LADC; ③Targeting PTGES combined with PD-L1 antibody therapy can inhibit immune escape. We will explain that PTGES regulates the spontaneous and metastasis of LADC through the AKT/G-CSF signaling pathway, and can open up new ideas for inhibiting the spontaneous and metastasis. This will elucidate its clinical translation significance and provide a new strategy for the classification, prognosis judgment and treatment of LADC.
肺癌的复发和转移是癌症死亡的重要原因,免疫治疗是目前最为有效的治疗方法,然而其有效率也仅有20-30%,背后的机制复杂不清。很多证据表明,肺组织非可控炎症可以促进肺癌的发生和肺转移,但具体机制不详。GPRC5A是肺抑癌基因。Gprc5a-敲除(ko)小鼠不仅能导致产生自发性肺肿瘤,且易于形成更多的肺转移,这提示,Gprc5a缺失促进转移前生态位(pre-metastatic niche, PMN)的形成。相比野生型小(WT)鼠,Gprc5a-ko小鼠对炎症信号如LPS异常易感,LPS诱导的PMN明显增强。通过表达谱和组学分析发现,这些表型与炎症信号PTGES/PGE2通路的异常性增强密切相关。实验表明,用PTGES抑制剂CAY10526预处理,可以明显抑制Gprc5a-ko小鼠肺组织PMN的形成。这说明,炎症信号促成的PMN依赖于Ptges/PGE2信号通路;也说明,PGE2是诱导免疫抑制微环境的形成的关键性信号。用anti-PD1免疫治疗WT小鼠肺转移,可以获得较好的疗效。然而,相同方法却不能治疗好Gprc5a-ko小鼠的肺转移。这说明,Gprc5a-ko小鼠肺组织呈免疫抑制微环境状态。用PTGES抑制剂CAY10526联合anti-PD1免疫治疗可以极大提高Gprc5a-ko小鼠肺转移的治疗疗效,获得了协同治疗效果。而单独用CAY10526或anti-PD1治疗都不能获得抑制肺转移的效果。分析表明,靶向PGE2信号通路,可以阻滞免疫抑制细胞的招募,增加T细胞浸润,重建免疫监督微环境,增强anti-PD1治疗疗效。详细作用机制仍在进行中。总之。PTGES/PGE2信号是肺组织PMN形成和免疫抑制微环境的关键信号;靶向PTGES/PGE2通路联合免疫治疗可以极大提高肺转移的治疗效果,为临床转化应用提出新方案。
