痛觉超敏的发生机制是未解之谜。申请者在前期发现的脊髓痛觉超敏闸门(Yan Lu, 2013)基础上,应用CRISPR/Cas9技术构建了PKCγ-tdTomato等系列基因编辑小鼠,确证了脊髓痛觉超敏闸门回路中PKCγ神经元接受Aβ纤维介导的先兴奋后抑制的双向调控,为研究此闸门功能提供了电生理闸门模型。本项目基于最新预实验提出该闸门开启机制假说:神经损伤后脊髓背角雌激素水平升高,激活Gly神经元上的雌激素受体GPR30,通过第二信使途径激活Gly神经元的L型Ca2+通道,再激活Kca,引起Gly神经元超极化,导致前馈抑制功能降低,PKCγ神经元被激活并上传非伤害性信息到痛觉通路,诱发痛觉超敏。本项目拟以系列转基因小鼠为研究对象,联合应用双膜片钳、光遗传学和脊髓光纤钙成像等技术,深入研究GPR30在脊髓Gly-PKCγ“痛觉超敏闸门”开启中的作用,为阐明痛觉超敏机制提供结构和功能基础。

The mechanism of neuropathic allodynia in the spinal cord has been a mystery. On the basis of the previously discovered spinal “allodynia gate” (Yan Lu, 2013), the applicant constructed a series of gene-editing mice, such as PKCγ-tdTomato, GlyT2-iCre-tdTomato, by using CRISPR/Cas9 technique, and testified that the spinal PKCγ neurons receive biphasic modulation from excitatory Abeta fiber input and inhibitory input driven by the Abeta input, which provided an “electrophysiological gate model" for studying the function of allodynia gate (Wang Qun, 2020). Based on our previous results and preliminary data, we hypothesized that the opening mechanism of “allodynia gate” is that the increased content of estrogens (E2) in the spinal cord following nerve injury activates the L-type Ca2+ channel of Gly neurons through the activation of E2 receptor (GPR30) and second massage pathway in Gly neurons, and then in turns activates Kca, leads to hyperpolarization of Gly neurons, decreases the release of Gly, alleviates the feed-forward inhibitory function, and eventually activates PKCgamma neurons and uploads the non-nociceptive information to the pain pathway via PKCγ neurons, and finally induces allodynia. By using dual patch clamp, optogenetics, spinal calcium image techniques, morphology, molecular biology, pain behavior, and take cell-specific GPR30 receptor knockout mice and various transgenic mice as a platform, the present study will investigate the role of GPR30 in the Gly-PKCγ feedforward inhibitory circuit of the spinal dorsal horn, to provide the structural and functional basis for elucidating themechanism of allodynia.
疼痛是一种让人不愉快的情感体验,它与组织损伤或潜在伤害有关,是许多疾病的主要症状,而慢性疼痛本身就是一种疾病。在慢性痛类型中,神经病理性疼痛症状重且治疗手段有限,是疼痛医学领域亟待攻克的重大课题。痛觉超敏指触觉等非痛刺激诱发疼痛,是人们用来解析病理性疼痛机制的典型症状。脊髓作为疼痛传导的中继站在其中发挥至关重要的调控作用。在经典疼痛闸门模型的基础上,本课题组发现了脊髓背角的Gly-PKCγ“痛觉超敏闸门”,但在病理情况下闸门开启,导致痛觉超敏的具体分子机制一直是未解之谜。在整个疼痛信号传导通路上,雌激素受体广泛分布。作为重要的神经调质,雌激素(Estrogen)已被确切证实在大脑中可通过调控突触可塑性从而影响认知行为,而突触可塑性改变亦是慢性疼痛的重要机制。长期以来雌激素即被证实对疼痛产生重要影响,但它发挥镇痛还是促痛作用一直存在争论。脊髓可以通过芳香化酶途径快速合成雌激素。Zhang Yuqiu课题组在小鼠鞘内注射强效天然雌激素17β-雌二醇(E2)后,发现可以诱导出无性别差异的疼痛症状,他们还证实了E2主要是通过膜受体介导的非基因组快速作用来改变脊髓背角突触传递,从而调控疼痛。有证据表明雌激素的三种主要受体中,GPR30(G protein-coupled receptor 30)是E2在脊髓背角产生快速致痛作用的关键受体,但是目前没有研究对其调控疼痛的环路特异性以及下游机制进行阐明。本课题应用多种技术,聚焦于脊髓背角“痛觉超敏闸门”回路,探讨神经损伤后在痛觉超敏形成过程中脊髓背角GPR30发挥的作用及其机制,以期阐明痛觉超敏闸门的开启机制,为精确解析神经病理性疼痛做出贡献,为后期新型镇痛药物靶点的开发提供结构和功能基础。
