临床中磨玻璃结节肺腺癌浸润前病变与浸润性病变的诊疗方式及预后大不相同,亟待术前能够精准判别,传统影像学检查存在一定的局限性以及人为判断的主观性,目前尚缺乏客观精准判断二者的方法。申请人前期利用放射组学特征对磨玻璃结节肺腺癌浸润性进行鉴别等原创性研究,但该模型需要进一步优化且其具体病理细胞微结构基础有待进一步探索。研究团队利用超高分辨荧光显微成像技术打破了光学的衍射极限,可以对亚百纳米的细胞结构进行3D成像、重建及可视化,为本研究提供新的机遇。鉴于此,本研究提出假设:磨玻璃结节肺腺癌瘤周组织细胞微结构存在差异,放射组学可挖掘深层图像信息特征反映该差异。本研究拟通过磨玻璃结节瘤周组织放射组学特征构建并进一步优化肺腺癌浸润性的预测模型,从而为临床诊疗提供辅助工具,并通过超高分辨三维成像技术明确磨玻璃结节肺腺癌瘤周组织细胞微结构差异,也可能为今后研究肺腺癌发生发展和浸润提供新的思路。
英文摘要
Clinical diagnosis and treatment of lung adenocarcinoma appearing as ground-glassnodules are quite different due to the different prognosis of patients. Because of the limitations of traditional imaging examinations and the subjectivity of human judgment, there is still a lack of accurate and objective method of diagnose. Our previous research found that radiomic features extracted from peritumoral parenchyma can be used to differentiate the invasiveness of adenocarcinoma appearing as ground-glass nodules. But the model needs further optimization and the basis on microstructure of cells needs further exploration. The research team break the limit of optical diffraction by the super-resolution microscopy and can perform 3D imaging, reconstruction and visualization of sub-hundred nanometer cell structures,which providing new opportunities for the study. Therefore, a hypothesis is put forward in the study as follows: There may exist differences in the microstructure of cells around the lung adenocarcinoma in ground-glass nodules, and radiomics can extract deep information of images to reflect the difference. This study intends to construct and optimize the prediction model of lung adenocarcinoma based on the radiomics features of ground glass nodules, and provide an auxiliary tool for clinical diagnosis and treatment. Meantime, it may clarify the difference in the microstructure around the lung adenocarcinoma of ground-glass nodules by the super-resolution microscopy, which may provide new ideas for future research on the development and invasion of lung adenocarcinoma.
肺癌是中国乃至全世界范围内的死亡率和发病率最高的恶性肿瘤。大多数肺癌诊断及预后评估尚需进一步提高,基于CT影像如能够无创性对肺癌进行精准诊断和预后评估,有助于指导临床治疗方案,改善患者的预后。前期我们的研究发现基于瘤周组织的影像组学特征能够帮助鉴别表现为磨玻璃结节的肺腺癌浸润性病理类型,并比较2mm及5mm瘤周组织特征的鉴别效能,辅助临床决策。基于此本课题中我们主要围绕瘤内及瘤周组织特征、影像组学特征及生物学可解释性进行深入分析。首先调研总结出目前三种瘤周划分方式,从不同的瘤周组织中提取的特征进行建模能够对有不同的鉴别及分层的效能,为我们后期进行建模提供了前期基础。为了进一步探究影像组学特征生物学意义,我们进行了肺癌患者预后的影像组学和深度学习特征的生物学相关性研究的系统评价,发现研究中通过基因集富集分析以及与基因、转录组学等相关性分析等方法,以揭示这些影像组学或深度学习特征在肺癌预后中的潜在生物学关联。研究基于单样本基因富集分析获得的免疫相关差异表达基因构建了肺腺癌风险预测模型,并联合临床特征绘制了列线图,列线图1、3、5 年总生存期的AUC分别为0.789、0.763、0.746,具有较高的预后风险预测效能,可为临床医师判断肺腺癌患者总生存期提供辅助工具。研究进一步评估结合影像-基因组学模型在预测肺癌患者预后中的有效性,并发现其能够提高模型的预测性能。此外,本研究负责人积极国内外合作,利用多中心数据,研究瘤内、瘤周组织特征与临床特征融合构建非小细胞肺癌预后的联合预测模型,提高患者预后评估。
