移植肾间质纤维化是影响移植肾长期存活的关键因素,机制至今不明。我们已证实肾血管EndMT在移植肾间质纤维化形成中起重要作用;近来又发现自噬可通过调控肾小管EMT减缓移植肾间质纤维化进展;而慢性移植肾失功患者移植肾血管内皮细胞中又出现线粒体自噬、细胞凋亡与Rictor/mTORC2通路异常。但线粒体自噬-凋亡平衡在肾血管EndMT形成中的作用迄今未见报道。据此我们推测:线粒体自噬-凋亡平衡可能通过调控肾血管EndMT进而在移植肾间质纤维化形成中起关键作用。为验证假说,本课题拟通过同种异体肾移植、内皮细胞ATG7或Rictor基因敲除小鼠及人血管内皮细胞模型,探讨线粒体自噬-凋亡平衡在肾血管EndMT及移植肾间质纤维化中的作用,并深入研究Rictor/mTORC2通路在其中的作用及机制。本课题预期成果将为从调控线粒体自噬-凋亡平衡和肾血管EndMT角度研发防治移植肾间质纤维化的措施提供新靶标。

Renal allograft interstitial fibrosis is a key factor affecting the long-term survival of transplanted kidney, and the mechanism is still unknown. We have confirmed that renal vascular endothelial-to-mesenchymal transition (EndMT) play an important role in the formation of renal allograft interstitial fibrosis. Recently, we found that autophagy can slow down the progression of transplanted renal interstitial fibrosis by regulating renal tubular Epithelial-Mesenchymal Transition (EMT) formation, and we also found that abnormal mitophagy, apoptosis and Rictor/mTORC2 pathway are present in transplanted renal vascular endothelial cells of recipients with chronic renal interstitial fibrosis. However, the function and mechanism of mitophagy-apoptosis balance involved in transplanted renal vascular EndMT have not been reported so far. On these grounds, we speculate that the mitophagy-apoptosis balance maybe play a key role in the formation of renal allograft interstitial fibrosis through regulating transplanted renal vascular EndMT. This project intends to adopt allogeneic kidney transplantation mice and conditional endothelial cells ATG7 or Rictor gene knockout mice, human vascular endothelial cells model to explore the actions of mitophagy-apoptosis balance in the formation of transplanted renal vascular EndMT and renal allograft interstitial fibrosis. On the basis of this, the role and mechanism of Rictor/mTORC2 pathway in this process will be discussed in-depth. The expected results of this project will provide new targets for the development of measures to prevent and treat renal allograft interstitial fibrosis from the perspective of regulating mitophagy-apoptotic balance and renal vascular EndMT.
背景:慢性移植肾功能不全(CAD)的主要组织病理学特征是移植肾间质纤维化和肾小管萎缩(IF/TA)。既往研究显示:肾血管内皮细胞-间充质细胞转分化(EndMT)是CAD患者中IF/TA病理过程的重要促进因素,然而其发生发展机制仍不明确。本研究旨在探讨线粒体自噬-凋亡平衡在肾血管EndMT发生及移植肾间质纤维化形成中的作用及机制;并且在此基础上,进一步探索Rictor/mTORC2信号通路活性对移植肾间质纤维化形成过程的影响及相关机制。.重要结果:首先,在CAD患者及慢性移植肾间质纤维化小鼠移植肾组织中,我们发现细胞凋亡水平增高及线粒体自噬活性增强,同时存在肾血管EndMT现象。在HUVECs中,我们发现TNFα可诱导EndMT发生,并增强细胞凋亡水平及线粒体自噬活性。进一步研究发现,TNFα不仅可通过调控NF-κB通路活性诱导EndMT发生,还能通过调控HIF-1α/BNIP3通路活性增强线粒体自噬活性。同时,我们还证实在HUVECs中敲低BNIP3表达可减弱TNFα诱导的线粒体自噬活性增强,并加重其介导的细胞凋亡水平及EndMT现象的变化。而进一步体外及体内实验也证实,降低血管内皮细胞中BNIP3表达可通过抑制线粒体自噬活性及增强细胞凋亡水平、进而促进EndMT发生并加重移植肾间质纤维化进程。随后,我们发现在纤维化的移植肾组织中Rictor/mTORC2通路被显著激活。在HUVECs中,抑制Rictor/mTORC2通路活性可减轻TNFα诱导的EndMT发生,而激活Rictor/mTORC2通路则可加重TNFα诱导的EndMT现象。此外,在同种异体肾移植小鼠模型中,特异性敲除内皮细胞中Rictor基因也能显著改善肾血管EndMT及移植肾间质纤维化进程,同时还可增强线粒体自噬活性及降低细胞凋亡水平。进一步研究发现:HUVECs中Rictor基因敲除能够增强BNIP3介导的线粒体自噬及减轻细胞凋亡水平;且抑制Rictor/mTORC2信号可通过降低E3泛素连接酶——MARCH5的表达,减少BNIP3的K130位点与泛素的K48链结合,进而抑制BNIP3蛋白的泛素化降解,从而增强BNIP3介导的线粒体自噬。此外,我们在体内外实验中也证实:敲低BNIP3可逆转因Rictor基因敲除引起的线粒体自噬增强、细胞凋亡水平下降、及肾血管EndMT和移植肾间质纤维化的影响。
