慢性前列腺炎/慢性骨盆疼痛综合征(CP/CPPS)是50岁以下男性最常见的泌尿系统疾患,其特征是会阴及骨盆区域的疼痛、尿频和性功能障碍。自身免疫诱发CP/CPPS的理论越来越受到重视。我们的前期研究发现包皮过长人群中CPPS患者和正常人群的包皮内板上的菌群存在明显的差异,并且CPPS患者的包皮内板菌群可以诱发NOD小鼠形成自身免疫性前列腺炎的相关症状,在这个过程中CD4 T细胞家族中的Th1、Th17等细胞有着重要的作用。包皮内板菌群的差异可以导致CD4 T细胞的激活和IL17、IFN-γ的分泌,在前列腺中形成自身免疫引发慢性炎症,进而引起症状发生。本课题拟用免疫表型分析、流式细胞术和16s-rRNA高通量测序等方法进行研究,探索包皮菌群通过激活CD4 T细胞在前列腺中分泌的IL17、IFN-γ致使CPPS症状发生的过程。这一课题对阐明包皮菌群对自身免疫性CPPS的发生机制有着重要意义。

Chronic prostatitis/chronic pelvic pain syndrome CP/CPPS is the most common urinary system disorder in men under 50 years of age. The theory that autoimmune theory induces CP/CPPS is getting more and more attention. In our previous study, there was a significant difference in the flora of the foreskin of CPPS patients and non-CPPS normals in the prepuce population, and the foreskin flora of CPPS patients can induce autoimmune prostatitis in NOD mice. In combination with literature and previous experiments, we found that Th1, Th17 cells in the CD4 T cell family and the immune factors secreted by them play an important role in the autoimmune. Differences in the bacterial flora of the inner foreskin can lead to the activation of CD4 T cells and the secretion of IL17 and IFN-γ, and the formation of autoimmunity in the prostate leads to chronic inflammation, which then causes pain in the perineal and pelvic areas. In this study, immunophenotypic analysis, flow cytometry, 16S-rRNA high-throughput sequencing and other methods were used to explore the process of the occurrence of CPPS symptoms caused by the foreskin flora through the activation of IL17 and IFN-γ secreted by CD4 T cells in the prostate. This study is of great significance to elucidate the mechanism of the occurrence of puce bacteria on autoimmune CPPS.
本课题研究组发现,包皮过长的CP/CPPS患者和无CP/CPPS病史的正常人包皮内板菌群存在菌群差异。包皮过长的CP/CPPS患者包皮内板定植的菌群相对于无CP/CPPS病史的正常人包皮内板菌群而言,能够诱导NOD/ShiJ (NOD)小鼠出现CPPS相关症状,进一步提取小鼠组织研究发现,CD4+ve和IL17+ve细胞在感染小鼠前列腺组织中均显著增加,CD4+ IL17细胞在局部增加NOD小鼠前列腺对包皮菌群抗原的反应,这可能是CPPS发生的关键因素。另外,前列腺组织中IL7显著增加小鼠CPPS疼痛症状的产生,且抗IL17治疗使IL7水平恢复到对照动物中观察到的水平。通过QRT- PCR评估了总前列腺样本中IL7Ra/CD127的水平明显上升,说明了IL7在NOD小鼠CPPS疼痛发生中起到的关键性作用。此外,蛋白质微阵列技术显示免疫的NOD小鼠前列腺组织中IFNg、IL-17A、IL-12p40和IL-12p70的表达水平较高,而免疫的C57BL/6小鼠前列腺组织中IFNg、IL-17A、IL-12p40和IL-12p70的表达水平较低。我们的实验结果表明,PI菌种定植后,NOD小鼠的炎症细胞因子、趋化因子、黏附分子、血管内皮生长因子和血管内皮生长因子的表达显著增加,这可能是一部分CPPS患者发病的始动因素。该研究成果提示早期切除包皮可能会降低部分CPPS患者发生机率,并且可能提供治疗慢性前列腺炎/慢性骨盆区域疼痛综合征这一疾病的相关药物。
