Nanoknife ablation is widely used in the treatment of hepatocellular carcinoma (HCC) based on the principle of irreversible electroporation (IRE) , however, the tumor recurrence rate after IRE is high. The applicant has uncovered that IRE can activate CD8+ T cell anti-tumor immunity at the early stage (Cancer lett 2021), nevertheless, the positive immune effect fails to completely inhibit the recurrence, and the underlying mechanism remains unknown. Through the preliminary experiments, the applicant found an increased infiltration of the myeloid-derived suppressor cell (MDSC) in the recurrent HCC after IRE, while the level of IFN-γ+CD8+ T cells is significantly decreased. Blocking MDSC by the neutralizing antibody can reduce the tumor relapse. Further researches revealed that the expression level of chemokine CCL2 that recruits MDSC is increased after the insufficient IRE, accompanied by the elevated level of mRNA m6A modification and its catalyzing enzyme METTL3. METTL3 knockdown can inhibit CCL2 translation and reduce the MDSC infiltration. This project aims to answer the key scientific question “How does the IRE regulate MDSC to promote the HCCrecurrence via m6A modification?”, hence verifying the hypothesis that “The IRE ablation upregualtes the METTL3 expression in the HCC cells to increase the m6A modification of CCL2 mRNA, which enhances the CCL2 translation and triggers the MDSC aggregation to inhibit the anti-tumor immune response, thus resulting in the HCC recurrence”. This project intends to clarify the immunosuppressive mechanism leading to the HCC recurrence after IRE ablation, and to provide new ideas for preventing and treating the HCC recurrence.
不可逆电穿孔消融(IRE)是治疗肝癌的新型消融技术,具有不损伤附近正常胆管、血管的特点,在临床上应用广泛。然而IRE消融后肝癌复发率高,限制其疗效进一步提高,至今缺乏有效干预措施,机制不清。课题组在青年项目资助下开展深入研究,1)发现IRE消融后MDSC浸润增加并与肿瘤复发生长相关;2)明确CCL2是IRE诱导的MDSC募集的关键因子;3)揭示METTL3可调控IRE后CCL2的翻译水平进而提高其表达;4)验证METTL3敲低可有效减少 IRE 消融后 MDSC 浸润;5)证实靶向 METTL3 可以有效减少不全 IRE 引起的肿瘤生长。这些试验数据有力证实了我们的科学假设:不全IRE通过上调肝癌细胞METTL3表达,增加CCL2的mRNA m6A修饰,从而促进其翻译来募集MDSC抑制抗肿瘤免疫,导致复发。为防治肝癌消融后复发提供了新靶点。相关成果先后发表于亚太胃肠肝病学会会刊Journal of Gastroenterology and Hepatology期刊(IF:3.7;共同通讯作者,标注基金资助)和美国细胞和基因治疗协会会刊Molecular Therapy期刊(IF:12.1;第一作者,标注基金资助)。参与培养3名博士研究生和一名硕士研究生,本人入选由国家传染病医学中心和中国门静脉高压联盟联合发布的“青柑学者”项目,是当年全国唯一入选者。
