氧化应激损伤和细胞凋亡在急性一氧化碳中毒(ACOP)脑损伤的发病中起重要作用。核转录因子红细胞2相关因子-2(Nrf2)是抗氧化应激Nrf2/ARE通路中的关键分子,其活性直接调控机体的抗氧化能力。申请者前期研究表明,ACOP大鼠脑组织中Nrf2蛋白低水平表达,细胞凋亡增加。应用莱菔硫烷(SFN)或脑内注射负载Nrf2基因的慢病毒均能增加Nrf2蛋白表达、减少细胞凋亡,减轻ACOP脑损伤。但其缺陷为:脑内药物蓄积浓度不足,药物半衰期短(仅2.2h),脑内注射创伤大、易并发感染等。因此,制备高通透性、长循环脑靶向药物是关键。本研究拟用改进的双乳化法制备长循环SFN-mPEG-PLGA纳米粒,用转铁蛋白受体单克隆抗体OX26对其功能化修饰后,通过体内外实验探讨Nrf2/ARE通路相关分子在ACOP脑损伤中的作用,明确该纳米粒治疗ACOP脑损伤的分子机制及可行性,为脑内靶向治疗提供理论依据。

Oxidative stress injury and apoptosis play an important role in the pathogenesis of brain damage induced by acute carbon monoxide poisoning (ACOP). As a key signal molecule in the nuclear factor erythrocyte two related factors-2 (Nrf2)/ARE pathway of antioxidant stress, the active status of Nrf2 directly regulates the antioxidant capacity of the body. The results in our previous studies indicated that the expression level of Nrf2 protein decreased, and apoptosis of neurons increased in brain tissue in ACOP rats. The administration of either Sulforaphane (SFN) or the intracerebral injection of lentivirus loaded with Nrf2 gene could increase the expression of Nrf2 protein and reduce neuronal apoptosis, and obviously alleviate brain damage after ACOP. Nevertheless, the methods mentioned above have the disadvantages including insufficient concentration of drug accumulation in brain tissue, too short half-life period of drug action (only 2.2 h), large trauma and high risk of intracerebral infection caused by local injection. Therefore, a key issue is to develop the brain targeted drugs with high permeability and long circulation. In the present project, the long-circulating SFN-mPEG-PLGA nanoparticles will be prepared by an improved double emulsification method, and its relevant parameters were characterized after functional modification with the transferrin receptor monoclonal antibodies OX26. We aim to explore the function of Nrf2/ARE pathway- related molecules, clarify the molecular mechanism and feasibility of the nanoparticles modified with OX26 in the treatment of brain injury caused by ACOP in vitro and in vivo, and thus providing a theoretical basis for brain targeted treatment in ACOP.
一氧化碳(CO)中毒是许多发达国家及发展中国家最常见的意外致死原因,其中急性重症CO中毒的死亡率可超过 50%。CO中毒可引起多种神经病理学改变,如急性脑损伤和迟发型脑病,严重影响患者的预后及生存质量。但是迄今为止,其发病机制尚不清楚,临床上也无确切可靠的治疗方法。因此,深入探讨CO中毒脑损伤的发病机制、寻找新的治疗靶点对修复受损的脑组织结构和功能、改善重症中毒患者的神经功能尤为重要。. (1)研究组应用高压氧舱二次染毒法成功制作急性CO中毒动物模型及细胞模型。该模型为课题组首创,具有制备简单、方便实用、造模成本低、成功率高、可控性强、容易复制等特点。. (2)研究组通过SFN与CO中毒网络药理学研究和脑损伤生信分析,发现SFN可能通过多种途径发挥治疗作用,包括抗氧化应激、抗炎、促进神经细胞再生等。 其中Keap1/Nrf2和NMDARs/CaMK通路是两条非常重要的信号通路,Nrf2和CaMK为信号通路上两个重要靶点。. (3)研究组成功构建并表征了功能性修饰的负载Tf-SFN的PEG-PLGA纳米粒,证明了其对PC12神经细胞的靶向性。这为纳米药物在治疗脑损伤中的精准输送提供了可靠的载体。该成果已申请国家发明专利并授权(专利号:202310826253.3)。. (4)研究组用细胞实验表明,功能性修饰的Tf-PEG-PLGA NPs对PC12神经细胞具有良好的生物相容性和生物活性,并且具有显著的细胞摄取能力,更高的神经细胞靶向力,为药物在神经系统中的释放提供了有效的途径。. (5)研究组通过动物模型实验证明,功能性修饰的负载SFN的纳米粒对一氧化碳中毒动物模型具有明显的干预作用,能够有效减轻CO中毒引起的脑损伤。该项目为后期提供了坚实的理论和实验基础。. 本研究已发表高水平医学论文9篇,核心期刊论文4篇,做大会报告2次,申请并授权国家发明2项。培养硕士研究生3名,博士研究生2名。
