Recent advances in the life sciences, in accordance with the innovations in various fields of science, realized the surgical procedures including lobectomy and pneumectomy with bronchoplasty, and lung transplantation for progressive irreversible pulmonary diseases, which allowed us the functional preservation or improvement of pulmonary function after surgery. The first surgery of lung transplantation was performed by Dr. James Hardy from the University of Mississippi in 1963, and Dr. Joel Cooper subsequently reported the monumental success of his clinical series of this procedure in 1983. 1, 2 Since then, an increasing number of lung transplantations have been performed mainly, in the United States. According to the Internal Society for Heart and Lung Transplantation (ISHLT) Registry report in 2006, as many as 21 265 patients were reported to undergo lung transplantation, with the 5-year survival rates around 50% in patients both with single-lung transplantation and with double-lung transplantation. 3The authors have participated in the basic research on lung transplantation for more than 30 years and had been looking for an opportunity for its clinical application. However, the special social situation in Japan, mainly because of the incidence of heart transplantation in 1968, did not allow us to perform transplantation from a brain-dead donor in our country for a long time.
