To define the prevalence of murine typhus in Mexico City, a serosurvey was performed in a population of healthy adult blood donors from the blood bank of the National Institute of Paediatrics in Mexico City. Two groups of serum samples were studied. The first group (n=100) was tested by an indirect fluorescent antibody (IFA) method for the identification of total immunoglobulin against Wilmington strain ofRickettsia typhiby indirect immunofluorescence. The second group (n=1282) was first screened with the Weil-Felix agglutination test. Samples in both tests were considered positive if they had a titre >1:80. From the first group 14 (14%) were positive by IFA from the second group, 193 (15.1%) people of both sexes were positive to the Weil-Felix test. The rate of seropositivity was statistically different for the 2 sexes (2-tailed χ2=3.33, 95% CI=0.037-0.14,P<0.05) since 49 (20.1% of the total female population) and 144 (13.9% of the total male population) tested positive by Weil-Felix. From these 193 positive serum samples, 90 were randomly selected and tested with IFA, out of which 88 were confirmed positive. The statistical difference in the frequency by sex was maintained after IFA testing. After adjusting for false positives, the rate for women was estimated to be 19.7% and for men 13.4%. The prevalence of antibodies againstR. typhiin healthy adults of both sexes from Mexico City was 16.7%.
