Anthony M Dart and Xiao-Jun Du here is substantial evidence that the sympathetic nervous system is activated during myocardial T ischaemia and that catecholamine release is important in determining the course of the ischaemic episode, particularly with respect to the occurrence of malignant arrhythmias.'" Many patients are already taking medication at the time of an ischaemic episode, and others are prescribed pharmacotherapy in the course of such an event. During the last few years, it has become clear from experimental studies that in many cases descriptions of drug properties derived from observations made under normal physiological conditions fail to allow for changes resulting from use under pathological circumstances. This review highlights some of these differences for both cardiovascular and non-cardiovascular drugs in relation to sympathetic neurotransmitter release in the ischaemic heart. Myocardial ischaemia induces postsynaptic changes (for example on ci and p adreno~ eptors,~ with altered myocardial responses to adrenergic stimulation) which may also alter the effects or potencies of various drugs. However these aspects of sympathetic neurotransmission fall outside the scope of this review.
