Unilateral neglect (UN) can affect many aspects of daily living. We propose a functional scale, designed to assess functional consequences of UN in real-life situations, consisting of ten items (such as dressing, washing, eating, communication, explorating activities, moving around): the CB scale (CBS), from the name of one of its authors. The severity of UN is scored for each item on a 4-point scale. Eighteen patients have been studied. Inter-rater reliability was good. The CBS score correlated well with clinical tests of UN, and with the Barthel index. Awareness of deficits was assessed by questioning the patient about his difficulties on the ten items of the CBS. A score of anosognosia was calculated (difference between the observer's score and the patient's self evaluation). This anosognosia score was well correlated with the severity of UN. We conclude that this scale is a simple, reliable and valid evaluation of functional consequences of UN in real-life situations and of anosognosia.
