A paper read at the 1st International Congress of Para-sitology, Rome, September 1964, where the author''s findings on the excretion of urinary [beta]-glucuronidase in cases of bilharzial infestation are examined. Increased bladder [beta]-glucuronidase excretion is associated with induced bladder epithelial tumors. It is also associated with active bladder bilharziasis. No increases were found in cases of intestinal bilharziasis or cases which had apparently lost the urinary infection. The increase in levels was shown to be due to the damage caused to the bladder epithelium by the passage of Schistosoma hemato-bium eggs. No carcinogens could be detected. In areas of Uganda, carcinogenic dietary factors have been found but these do not coincide with S. hematobium foci. Bladder cancers are relatively rare in some areas where S. hematobium is endemic, while in other areas the incidence of bladder cancer is high. It is concluded that S. hematobium infestation could only precipitate the onset of a bladder tumor by providing extra [beta]-glucuronidase into the bladder lumen so as to increase the hydrolysis of exogenous carcinogen glucuronides if present.
