In the beginning, Medawar discovered that blood transfusions immunized recipients against subsequent transplants (1). This view was almost universally held for 25 years–but, in 1973, findings contrary to this fundamental tenet of transplantation emerged in the setting of clinical kidney transplants. Following transfusions, transplant survival rates were not diminished–instead, inexplicably, survival rates were augmented (2). Though a multitude of studies have confirmed the phenomenon itself (3, 4), an explanation for this perplexing contradiction has eluded us. Now, 10 years later, we advance a new theory which we hope will unify the accumulated knowledge about transfusions.The hypothesis states that the primary function of transfusions is, indeed, to immunize patients and not to induce toler-ance, or to induce enhancement, or to serve as a donor selection mechanism (3). As expected from the beginning, transfusions immunize but immunosuppression eliminates the responding clones.
