For the past 4 years, we have correlated the clinical picture with the gross and microscopic appearance of tissue removed from patients who were seen with a mass in the epididymis. It soon became apparent that there was a great deal of confusion in the identification of epididymal tumors. In this paper, we have reviewed the literature and reported significant cases of our own. We have developed a classification of neoplastic and non-neoplastic masses arising in the epididymis which provides a guide to the diagnosis of diseases that occur in this structure. In 1951, Longo and Thompson stated that a total of 134 primary tumors of the epididymis had been reported in the world literature; of these, 74 per cent were benign. By far the most common benign neoplasm of the epididymis was the adenomatoid tumor (at times referred to as a mesothelioma) which comprised approximately 70 per cent of this group. This tumor has been reported not infrequently and was first described by Sakaguchi in 1916. Glenn in 1955 reported 3 cases, which at that time made a total of 123 in all. Adenomatoid tumors occur not only in the epididymis but in the testicular tunics, the serosal surface of the fallopian tube, uterus and ovary as well.Several theories of the origin of adenomatoid tumors have been advanced, ie that they are of mesothelial origin or that they may arise as hamartomas of the mesonephros or from mullerian vestiges in the epididymis. Other tumors occur less frequently. Leiomyoma is the second most common benign tumor of the epididymis. Longo and Thompson stated that they comprise 10 per cent of primary neoplasms of the epididymis. Fourteen cases were recorded in the literature up to 1951. Gordon-Taylor reported a fibromyoma of the epididymis in 1943. Lipomas have been reported as occurring in the epididymis, but this is doubtful as there is no fat in this structure, according to Gibson and
